Struck by... (Not Elsewhere Classified)

Employee removed the pin on the Index stacker and a rock fell in the hole. He stuck his finger in the hole and the wheel was turned and pinched the employee's finger.

Fall from machine

Employee had loaded the 60 crawler on the 101-M lowboy trailer, when he slipped and fell off the trailer.

Struck against a moving object

Prill truck 468 was traveling on Northwest ramp when the operator lost control and rolled the truck on it's side.

Over-exertion in lifting objects

Employee was unloading a 12 volt battery out of the back of his pickup, when he felt a sharp pain in his back.

2 employee's were rolling a drum that was filled with absorb-all and oil. As they rolled the drum, it tipped over and fell on one of the employee's. As the other employee lifted the drum off of him, he strained his back. The drum weighed approximately 400#.

Employee hurt his shoulder/neck area on 08/07/2008. Employee was lifting a pump to be installed on a haul truck and felt a pop in his left shoulder. He returned to work on 4/7/09. He re-injured himself on 7/15/09. He did not report until 7/16/09. The incident has been under investigation by GAB. The claim was accepted 11/18/09.

After being investigated, it was determined that the employee could have jammed his hand into the door handle, when exiting the men's restroom as someone was entering it. The results of the investigation were obtained on 08/6/2009 from GAB.

Employee was walking on the North MFL pad, when he stepped into a hole. When he stepped out of the hole, he heard a pop come from the back of his left ankle and felt numbness.

EE was at general office storage room. Had just sat down to take a sip of coffee and then decided to empty the mop water. As she stood up, she felt a pop in the left knee. She sat back down for 20 minutes. Decided to get up off the chair and felt a sharp pain. She kept walking down the hallway. It was hurting so bad she could barely walk.

Over-exertion in pulling or pushing objects

Employee hurt right shoulder while pulling power cable on 3/1/09. It was not reported to Safety until he went to the clinic on 5/2/09. After an investigation was completed, it was determined to be a work related injury.

Fall from machine

Employee was operating an ATV, when he hit a rock. The employee went forward over the handle bars and landed on his shoulder.

Fall onto or against objects

Employee was slewing the RS1 Radial Stacker on the North Mine for Leach pad, when he drove the stacker off the edge of the 3 foot lift.

Struck by falling object

Employee was working in Section ""J"" cell, line 30, when a blank cathode got hung up on a sleeper. The blank cathode came off of the strongback, landing on the employee's left foot.

Over-exertion (Not Elsewhere Classified)

Employee was initially injured on 6/19/2008 (E-Document #638715). Employee returned back to work on 1/28/2009. Went on his 7 days off on 02/12/2009 and then went to his personal doctor on 02/20/2009. A new 7000-1 is being submitted. We were not notified until 02/24/2009.

Fall from ladders

Employee was initially injured on 6/19/2008 (E-Docment #638715). Employee has been back to work full duty. On 1/22/2009 he was taken off work again (no new injury). A new 7000-1 is being submitted as per the MSHA definition of a pre-existing condition.

Struck against a moving object

Operator of the 610 haul truck felt pain in his lower back when he dismounted his haul truck at the end of his shift. He got loaded at the 41 shovel about 4:00 p.m. and was shook around a bit. He did not report it until 01/03/2009 at approximately 2:00 p.m..

Struck against a moving object

Employee was released with no restrictions on 9/9/09, after the 594 haul truck rolled backwards and struck a berm and rolled on its side and caught fire. Employee returned to clinic on 9/16/09 and was given restrictions.

Employee was leaving a chute on the North Stacker, when he slipped and hit his left hand, causing a fracture.

Employee was installing a guard on the 15B conveyor and the guard fell on his left foot, striking the top.

Caught in, under or between a moving and a stationary object

Employee was helping load forklift forks by hand into the back of a pickup. When they slid the fork in, the employee's left little finger was smashed, causing a compound fracture.

Employee was reaching into a hopper to remove boulders, when he felt a ""pop"" in his right lower leg.

Fall to the walkway or working surface

The employee went to unlock a control room door, when the rocks under his foot slipped out from under him and he hit his left knee on the cement pad.

Struck against a moving object

Employee's shift began at 7:00 p.m. on 1-9-2009. Employee was operating a service truck and rear-ended another truck.

Over-exertion in pulling or pushing objects

Employee was moving a 55 gallon drum of oil, when the drum slipped off of the pallet. Employee tilted the drum to remove his foot and felt pain in his lower back and right shoulder.

Over-exertion in lifting objects

After our investigation was concluded on 1/13/2010, it was determined that the employee strained his left shoulder, while helping carry a battery and it is an Industrial injury.

Employee was removing a cooling tube off of the 646 haul truck, when he felt some pain in his left shoulder.

The employee was struck in the inside of his right knee with a come-a-long, when it came off of the structure.

Employee was cutting off a bracket and moved in a different direction, causing him to feel a pull in his shoulder area. Employee refused treatment at time of incident. Employee reported on 9/11/09 that he would like to have his shoulder checked out at the clinic.

Employee was stepping off of the 11R drill, when he had lower back pain.

Employee was walking to her work area, when she slipped on the rocks and fell, striking her right knee on a rock. The area was slick due to rain.

Fall to the walkway or working surface

The employee was helping flush lines at the King 6230N dump, when a rock rolled from underneath his left foot and he fell, twisting his left ankle.

The operator of the 617 haul truck dumped out on the King 5150 dump. He reached over to the passenger door to put some trash in the trash bag that was hanging on the passenger door handle, when he felt pain in his lower back.

Over-exertion in wielding or throwing objects

Employee was removing picks on a 24H motorgrader with a 4# sledge hammer, when he felt pain in his upper back. He was given a full duty release on 08/04/2009 and then returned to the clinic on 08/09/2009, with a muscle spasm. He was given restrictions for the rest of the day and given a full duty release on 08/10/2009.

Employee was descending the last step of the crusher stairs, stepped on a rock on the ground and turned his ankle.

Struck against a moving object

The 596 haul truck was being loaded at the 15 shovel. When she received the first dipper of muck, she felt pain in her neck and back.

Over-exertion in lifting objects

Employee was under a belt, replacing an idler. While lifting the idler, employee felt a pain in his back.

Employee received a laceration to his finger when attempting to lower a nose cone liner from a shelf.

Struck against a moving object

Employee was operating a haul truck and was traveling down a ramp behind another truck. The truck in the lead stopped on the haul road, the haul truck following attempted to stop but went into a slide and made contact with the lead haul truck.

Struck against stationary object

Employee cut his hand while installing a cover on a breaker panel. The employee received 5 stitches.

Absorption of radiations, caustics, toxic and noxious substances

Top of safety shut off valve (on Vac Truck) came off and employee was putting valve back when acid sprayed him in the face and neck. He was wearing a face shield and safety glasses. He flushed eye and washed off head and neck at the showers. Went to Gila Health Resources and had eye flushed and was released to return to work.

Employee turned a wheel barrow right side up. He turned it over and received a splinter in his right hand. He was wearing cotton gloves.

Struck by falling object

Supervisor was washing the under carridge of the 28-O pickup, when a chunk of mud fell, splashing his left eye. Supervisor had on Safety Glasses.

Caught in, under or between a moving and a stationary object

Employee was trying to remove a support rod in the structure of the Central Tankhouse. After removing a bolt and nut, the rod would not come loose. The employee shook the rod, it came loose, pinning the employee's left pinky finger between the rod and the beam. Employee received 3 sutures.

Employee smashed his right middle finger while using a hydraulic ram to raise the bowl on the secondary crusher. The employee was repositioning the ram, when it slipped.

Employee reached to grab the sawall out of the Utility truck. The employee hit the switch and pinched the right index finger on the shaft cover of the sawall.
