While performing lifting operations of a liner, the rigging came loose causing the liner to flip. The liner began to swing, striking employee on leg and pinning against stand. Approximate weight was 7000lb.

Employee struck by end of 30"" diameter X 40' pipe while pipe being lifted. Truck had been moved into the work zone to allow for traffic to pass. Truck was not moved away and created a pinch point. Pipe shifted when lifted, striking employee, pinching employee between the pick-up truck and the end of the pipe, fracturing employee's pelvis.
Employee was prespotted at a shovel and the operator states the dipper made contact because the truck was too close. The operator said they were fine through the shift and woke up with back pain before the start of their following shift.
An employee was performing a pre-shift inspection when EE stepped on a rock and rolled EE's ankle causing EE to fall to the ground, landing on EE's knee.
While driving a pick up returning to the mine offices for shift change, a dozer operator states the pickup hit a pot hole. The EE did not feel anything was wrong and went home and did not report the event. When EE woke up for next shift, EE states EE felt pain in lower back, but came to work and performed assigned duties operating a bulldozer.
Employee was walking on uneven ground when foot slipped out from under EE, causing EE to fall and break ankle.
Employee was working at the Metcalf Mill South Repair Bay using shovel to push water to flex off of vacuum truck when EE put right foot in the flex, the employee panicked and sat down and the vacuum hose went up to EE's right calf and employee did have vacuum break as directed on inspection sheet. Employee was taken to the Urgent Care and released home.
An employee was climbing a ladder to work on the 563HT when their right foot slipped off the 3rd rung and they landed on their right foot causing a fracture.
Driver was initiating offload of sulfuric acid. Hose was not secured when driver opened the external valve resulting in driver being sprayed with sulfuric acid.
Employees were trying loosen a bolt on the 3370 regrind cyclone feed pump. As bolt came loose employee felt pop and crunching in left shoulder.
Employee was installing drip line, as the ATV was pulling the line. The drip line made contact with the employee causing EE to fall, resulting in a broken leg.
Employee was collecting solids down in the underflow launders when felt a sting in lower mid back. Employee didn't report the injury until 12/05/2018.
Compressor hose fitting struck employee in the face resulting in mandible fracture
Employee attempted to remove a fuel fitting while the fuel truck rolled away from the work area. The fuel hose became taught and the fitting broke loose, striking the employee.
Employee was using a pry bar to get a chute door back in place when the pry bar slipped.
Employee was pulling HDPE pipe with an ATV when the ATV struck a large rock. The ATV stopped abruptly and the employee hit knee on the fender.
A welder was taking off coveralls when the welder heard a tearing noise in arm.
Employee claims they stepped into water and got water in their boots. The company was not notified of this incident until 10/22/18 when the employee's spouse called the company Safety Superintendent stating they were in the hospital for an infection.
Employee was setting a hydraulic jack when they felt a pop and lost strength in their arm.
An employee was performing maintenance on an MCC when an arc flash occurred and the employee received burns to the face and lower arms.
An employee was descending from the 11 Shovel when safety vest caught on the swing gate causing them to lose balance and fall down the stairs resulting in bruising and cuts to the face.
Employee set an extension ladder up, upside down, with the hard plastic on the ground. Employee began to climb the ladder, got 7' up, and the ladder fell out from under ee.
A boilermaker/welder was performing a 2 man lift on an steel square tubing measuring approximately 18 feet and weighing an estimated 200lbs when the employee felt a mild pain on the hand. On 10/1/18 the employee visited the dr. for a follow up and was given restrictions making this a restricted duty injury.
EE was taking off the #2A unit dust collector to change out the 2A Short head. The EE had removed all the connecting bolts for the dust collector pipe with the exception of two, one at each end. The dust collector pipe fell approximately 4 inches hitting EE on EE's hardhat and pinning EE's left forearm between the wall, existing conduit with 24 inch dust collector pipe.
The operator of the 506HT was coming down the empty lane from the highway crossing to the crusher 3 haul ramp and hit potholes in the road causing back pain.
An employee was using the auger when it caught on the rocky material and twisted causing the employee's wrist to twist.
An employee alleges that on 5/13/2018 they were standing on a ladder and felt pain in their back.
EE was trying to find out why containment area of the HRC800 was filling up with mud and silt. EE was walking in the mud and reached to shut off a valve and when EE turned they felt a sharp pain in the left foot. Pain went away and 2 days later felt pain again and it was determined EE has sustained a hairline fracture to the outer portion of the left foot.
Employee was walking down the walkway along the 14-Conveyor belt and slipped forward on the walkway surface. EE felt knee lock up and grabbed the handrail to avoid falling.
On 2/16/2018 an employee was kneeling down and as EE stood up, they felt a pop in their left knee. On 3/15/2018, the employee was taken off work for surgery to repair a torn tendon in their knee.
At approximately 1308, an employee was assisting with removal of mast pins. The employee was holding a brass knockout for pin removal. While removing the final pin, the mast shifted and landed on the employee's right hand resulting in lacerations to the middle fingers. Employee was taken to the clinic and received 5 stitches.
An employee was attempting to manually raise the boarding ladder and felt pain in abdomen. On 3/1/2018, the employee received a confirmed diagnosis of a hernia.
An electrician was working when a grease line pressurized by an air pump blew striking the hand and covering employee in grease. The employee was evaluated and released. On 2/8/2018, the employee returned to work from scheduled days off. On 2/11/2018, the employee's duties were modified to accommodate the continued pain in the hand.
On 1-10-18 an employee was fusing pipe, when a fuse broke and raffinate started spraying. Employee moved quickly away when ee felt left knee pop. Felt ok, went home and started hurting. Came to work on 1-11-18, started hurting more called supervisor.
An employee was traveling to the dump when they hit a pot hole causing slight irritation in the lower back, When the employee dumped out, the haul truck bed slammed down causing more irritation. The haul truck was discovered to have a bad hoist cylinder which was replaced.

A mechanic was changing a shank guard when it slipped and struck the tips of an employee's fingers.
The affected employee pinned foot between a flange on a spool piece and an I-Beam fracturing the metatarsal bones on the right foot.
Employee claims that they slipped on rock pile, reported knee pain 11/1/18.

Employee tried to pick up the return idler (approx. weight was 135) from a pallet on the second tower and felt how heavy it was when they set it back down that is when they felt the pain. EE then had another employee help with a rope to assist to carry it up the two flights of stairs. They set it down on the upper deck. When EE tried to move arm they felt more pain.

EE was using left arm to push forward on wrench and hand slipped off the wrench making contact with the wall in the chute causing injury to left index finger.
An employee placed a green horn on the edge of the bed of the truck to clean crater from it. The green horn shifted and fell from the truck, striking the employee's foot causing a fracture to the 5th digit metatarsal.

The employee received 4 stitches.

On 9/7/2018, an employee got dust in EE's eye. After continued irritation, on 9/10/2018, the employee went to the clinic for additional evaluation and received a drop of medication in the right eye.

An employee was checking the seal while installing a bearing when the forklift operator side shifted and the employee's finger was pinched causing a fracture to the index finger.
Mechanic was installing a pin during a new molebord installation on 78 dozer. When the pin fell into place, the weight of the pin pinched the employee's right pinky finger between the flat retainer plate and molebord ear, resulting in a laceration.
Employee reached under the seat of a boom truck to adjust the seat position. While griping the adjusting handle, the mechanic moved EE's hand along the adjusting handle contacting a sharp edge and lacerating EE's finger open.
Employee stated trigger on air chisel was initiated by contact with scaffolding while hose and tool was being moved within the 401 tank at the CLP. Employee stated this caused air to release and dust was blown into employee eyes.
A carpenter was walking across a rebar mat inside a un-poured foundation and tripped and fell. When EE fell EE landed on EE's wrist and fractured a bone in EE's left wrist.

At around 7:25pm, the employee was exiting the LV719C truck from the front passenger side. As employee was about to shut the door, the wind caught the door and closed on employee's finger causing the very tip of finger to be severed, no bone was affected.
An employee reached under the seat of a haul truck and contacted a sharp piece of metal causing a minor laceration. The laceration was closed using steri-strips. On 1/5/2018, the employee started normal scheduled 7 days off.
