After investigation completed on 1/25/2012, it has been determined that an employee walking on the King 5320 stockpile felt a sharp pain in his left calf.
Employee's right foot was hit by a worn shell liner, while it was being removed.
Lower back muscle strain while installing trans pump.
Mine Electrician was burned after attempting to connect a pothead to an energized switch house.
Employee was struck in the face by a pnuematic/hydraulic ram while working on the 645 Haul Truck. Employee received stitches to nose and lost 4 teeth.
Employee fell at the 801 booster and hurt his shoulder. MRI and an Ortho Consult was requested. On 5/25/2011, the employee was given No Work Status by the Ortho Consult. Employee notified FMMO safety on 5/31/2011.
9:35 a.m. the employee began washing a F150 EMS Company vehicle and while walking toward the high pressure hose and valve to turn it on slipped on a icy surface. Employee fall on the concrete surface causing a left wrist fracture when trying to break the fall.
Employee was checking the C-scalping screen door, when he slipped and fell backwards, landing on his elbow. The employee returned to be evaluated on 10/28/2011. On 10/31/2011, he received restrictions and it was determined that he has an AC separation.
Pump Shop employee fell and his knee struck a pump stand.
Operator slipped and twisted right ankle.
Operator of the 84 Dozer slipped while dismounting dozer.
Employee smashed his left foot, when he opened the rear door and a brass heat sink (approximately 10#) fell on top of it.
Welder was descending a set of stairsteps. His foot (had crater on bottom of boot) slipped on a strip of concrete/metal, causing him to catch himself using his right hand/shoulder. The welder strained his right shoulder.
After our investigation, it has been determined that the injury that the employee received while shutting the van door will be claimed as a reportable injury.
Tech was lifting the turbocharger off of an engine onto the deck then to lower the turbo with his service truck crane. In doing so tech felt pain in his lower groin area and a small bulge in the lower groin area.

EE did not set grappler bucket on the ground it was suspended 2 to 3 ft above scrape steel a piece of steel fell out of grappler and hit the pile steel sending a small piece of steel into the finger of ee.
Injured person was standing on the passenger side of the vehicle. FMI employee was backing his vehicle (1 ton flat bed) up, was blinded by the sun coming over the local horizon. He struck our vehicle on the drivers side of the Western Industrial vehicle pushing it 4-5 inches causing the Western Vehicle to strike the injured party at the knee.
Employee's got knocked between forklift & truck tire. Empire EE was repairing unit 591 & was at his tool box near lt front tire. A FMX emp was operating a forklift, backed up to talk to Empire EE & hit gas not brake on forklift. He hit Empire EE's tool box & then knocked EE against tire. EE was complaining that his arm was sore so he went to the clinic.
Employee's foot was caught under his tool box while moving it.
Employee was hitting rod joint with a hammer to assist in breaking joint loose. The hammer glanced off of the drill pipe and hit his hand which was holding a breakout tool holder.
A Tire repairman's left foot fractured, when a hub was flipped over by a Heavy Duty Mechanic.
Employee was shoveling in the #23 ball mill pit, when she went over the gravel and her thumb made contact with the angle iron table stand.
Employee tripped and fell on the steps in the Concentrator rougher flotation area, causing lacerations to his forehead (above eye) and nose, receiving stitches.
Helper and driller were washing the crew truck at the drill site. The helper was using a higher pressure washer to rinse the truck and did not see the drillers hand. He accidentally sprayed the driller's hand, resulting in a puncture wound.
Employee states his left finger was pinched between the pin and wet splitter mount.
Employee got his right index finger caught between the door of the #4 ball mill and the wall of the mill, when they were re-installing the door on the mill. Employee received stitches for the cut.
Employee's hand was at the base of the tie while another employee was hammering the tie. The tube shifted off the tie and fell off onto employee's right open hand causing a laceration to the ring finger.
Employee smashed the tip of his 5th finger on his left hand.
EE was crawling under a desk carrying a scraper in his right hand to remove floor debris preparing to wax the floor. EE struck the scraper with his right knee causing the scraper to move striking his left thumb. The thumb sustained a 2 1/4 laceration treated with nine sutures.
Employee fractured the tip of his second finger on his right hand. Employee injured finger on 08/26/2011 and was Scheduled off on the weekend returning to work on 08/29/2011.
A Concentrator Maintenance employee received chemical burns to both legs.
Employee smashed his finger between the wrench and rim, when he was removing the oil plug on the right spindle of the 537 haul truck.
Employee was cutting a 3 inch HDPE pipe at the B-Hill laydown yard with a sawzall. The blade cut through the pipe and went through his boot, cutting his left foot between the 4th and 5th toe.
Employee was welding on a haul truck bed and received flash burns to his eyes.
Employee got something in his left eye after he was cutting on a spare D-11 push arm.
Cut to left index finger and received 8 stitches.
Employee was relocating a feedline and installing a blind flange, upon removal of the flange, residual pressure in the line splashed up into both eyes.

Employee was manually moving a single lead anode on a pallet; while pulling the anode to re-position it, the hanger made contact with the employee's right shin. The employee received stitches in his shin.
Employee was struck by a shackle breaking an upper tooth.
Employee was putting together a scaffolding system at the Southside Tank House, in the Morenci Mine, in Morenci, Arizona. While working at this location and installing a ""runner"" to the scaffolding, it popped out and struck his upper lip. This form was filled out on the date but the confirmation number was never obtained.
Employee was cutting a 1"" piece of pipe secured on a chain vise. The cutting wheel bound up and kicked back on the employee. The kick back caused the grinding wheel to contact the employees left arm resulting in lacerations.
