A fire was called in and upon arrival to the location of the fire, it was found a generator was on fire. The location was at Hoop's Hill Radio Towers.
Employees were cutting bolts from a feed plate on the ""C"" standard crusher. During the process, a piece of hot metal landed on the screen and started a fire.
Operator of the 527 haul truck reported that he hit a pothole and had neck and back pain and trouble breathing.
Employee's right hand was pinched between a spiral bar and a beam.
Employee was hit in the leg by a 16 inch pipe that was being moved by a loader.
Employee was tightening the radio antenna, when she received a shock.
Employee had cleaned office cubicles and began mopping the area. Employee mopped outward and as she mopped backward she tripped over the left wheel of equipment placed in the cubicle.
A ball mill trainee was climbing a small access ladder and the top of his hard hat hit a low hanging pipe that was sprayed yellow. Employee had pain in his neck.
EE was working on the original side of the MCC secondary crusher roof repairing water leaks around the air conditioner ducts. At approx 12:45 he claims as he was cleaning around the duct with a vacuum, he stood up and his right knee popped resulting in the alleged injury.
Employe lost his balance while descending a stairway with a 16 foot ladder in the #1 elevator sump area. Employee was given no restriction. Returned to clinic on 3/17/2010 and was given no restriction. Took the afternoon of the 17th off, due to his knee hurting him.
The employee was shoveling muck into a chute, when he felt pain in his stomach. After our investigation, it was determined that it is a work related injury.
The operator of the 544 haul truck was descending the lower steps, when his left foot missed the rung. He then fell to the ground.

This is an ""Alleged Injury"", this report may be amended apon further investigation. EE was stepping out of his truck to pick up equipment for the end of the day. EE stepped onto a round rock that gave away under him casuing him to roll out his left ankle, twisting it.
injured party was helping to remove a jammed strong back. When the strong back came loose the injured employee was pulled down by the weight of the strong back and strained his neck
The operator of the 14R drill was attempting to exit the cab of the drill, when her left foot got caught on the strap of her personal bag, causing her to trip. When she tripped her right knee hit the frame of the cab door.
Employees were pulling in a new belt on the 11E feeder, when the belt clamp came loose and hit the supervisor in the right leg, fracturing it.
Employee got his arm caught between a chain and sprocket on the head pulley of the apron feeder.
The 643 haul truck was in process of dumping out, when the haul truck went through the berm and rolled.
Employee was attempting to move a T-brace in the lay down area. She reached down with her right hand to pick up the brace, when she felt her right shoulder pop out of place.
Employee was carrying trash through a repair bay doorway and was watching equipment movement in front of the building. Employee walked forward stepping on the access ramp edge. Employee fell.
Employee stepped in mud while carrying 2 signs and tried to jump forward to keep from sinking in the mud, when her right bicep and shoulder/neck area began to hurt.

The operator of the 618 haul truck took a bathroom delay and when climbing back on his haul truck he felt a pop in his right wrist.
Mechanics were installing a sun gear on the left final drive of the 584 haul truck. 1 mechanic was pushing on the sun gear and asked the other mechanic to hit it with a dead blow hammer. On the third hit, the hammer made contact with the axle bar and deflected off, hitting the mechanic holding the sun gear on his left pinky finger.
The employee was being helped up on the berm to look over the dump. As she was stepping up, she slipped and fell. Her co-worker had a hold of her hand. After the MRI, it was determined that the employee has an avulsion fracture.
Employee was shoveling material spillage from the air splitter floor. He felt a pain in his left back area. Incident was under investigation, pending the results of the MRI and sonogram.

Employee was tightening up oil filters when he reached to shut off an oil hose that was on the floor the ratchet handle on the strap wrench hit him in the teeth and chipped several front teeth.
While working on the shake & brake crew on 5575 king, rafe built up on his safety glasses. As he pushed his safety glasses back on his nose rafe material entered his left eye resulting in injury.
The driver of the 625 haul truck was attempting to get out of his truck. He opened the door and as he pushed the door open the wind blew it closed, catching his left index finger between the door and the frame.
Employee was hit on the forehead with a rock thrown at him by another employee. Employee had just taken off his hard hat to get some air on his head.
Employee went to the 5150 Que point and got off his haul truck to use the restroom. A gust of wind blew dirt into his eyes. Did not go to the clinic until 02/27/2010 at the start of his shift.
