Employee was adjusting a jack pad on a crane outrigger when EE's hand was crushed.

Employee was carrying a bucket of tools next to their service truck parked near the 5 conveyor. LV675 drove past closely and struck the bucket in their hand causing it to swing. The employee reported to urgent care on 11/24/2023 stating that they have an injury.

Haul truck bed dropped while dumping load shaking up the employee.

Employee states hole deck cover on the drill was blown off by the wind striking employee's foot

Empire Employee was attempting to re-position a lifting strap. The Employee believed that the weight had been relieved from the strap and began to attempt to move the strap. The employee did not know that the strap was in a bind on a flange on the pipe causing the strap to pinch a finger, causing a laceration that required 5 stitches to treat.

Employee was stepping out of the transport van.

Employee was dismounting fuel truck when they stepped on a rock twisting their ankle.

FCX technician was operating a hydraulic hand pump to raise the crowd drive motor on a 4100XPC to make room for replacing the crowd belts. FCX technician states the hand pump lost pressure and the motor assembly fell. EE had left hand holding the jack screw in position when it fell and the jack screw pinched their left thumb and index finger against the block.

Employee shoulder started hurting after lifting a piece of 4"" HDPE pipe.

Employee had floatation mill process solution enter their rubber boots causing caustic burn.

Contracted employee took a turn to fast and tipped the truck and trailer over. As a result, they were trapped inside the truck and was going in and out of consciousness. Contractor was transported by ambulance to the local hospital with a slight brain bleed and concussion.

While EE was pulling EE's 5th wheel pin, EE slipped and fell over concrete barrier hitting EE's left calf as EE fell. EE's calf started hurting and swelling 30 minutes later.

Then, after blinking a few times, EE thought had gotten it out. On EE's way to terminal, it started to irritate eye and got worse. The driver stated did not report to the mine.

Another employee was moving the trolley along the monorail and trolley fell off of the end of the monorail striking the IE on the right-hand ring finger causing a fracture.

Haul truck operator states they hit a rough area on the road cause their back to hurt.

Employee was hit by skid-steer loader when it abruptly made a turn running over employee's leg.

When EE's left foot touched the ground, and EE put weight on it; EE's ankle buckled and gave out. This caused EE to fall backward. When EE put arm out to break the fall upon impact with the ground, EE hurt right wrist.

Employee fell approximately 5 feet after not making full contact with stair on 793 power stairs. Employee hit their shoulder on the concrete and was given restrictions and not able to utilize their right arm repeatedly.

This resulted in a split impact injury. First aid was administered and taken to the clinic where EE received 8 stitches, and 1 day restrictions.

As employee stepped over the mound, employee placed EE's foot on the far side of the mound, unbeknownst to EE, where a rock in the mound was located. The rock moved, EE lost balance and fell backwards, striking EE's head on Crew truck body.

Employee got slag in their eye when they tilted the welding helmet up.

Employee was cutting rope and cut left thumb at the base. Employee said they did not feel the cut until they took off the gloves to wash their hands for lunch. That is when employee called the supervisor.

Summer Intern was walking across parking lot when they were stung by a Wasp.

Employee states the dump handle of the haul truck slipped out of their hand causing the truck to shake violently.

Employee dropped a 4"" HDPE pipe on their own finger causing a laceration.

EE was removing a hydraulic line near the brake accumulator on an 834K with the cab removed. When the line came loose, pressure inside the line caused hydraulic oil to hit the EE in the face. The EE's safety glasses took the brunt of the impact but some hydraulic oil still got in the EE's right eye.

EE was cutting the CM Hose with a utility knife and hand slipped, slicing the left-hand ring finger. EE called the Triage Hotline number for support on dealing with the injury. EE cleaned the cut and applied Neosporin and a band-aid to the affected finger. EE was seen at Local Urgent Care and was informed that the cut would require stiches.

A SWI employee was working on the SWMFL pad, as the employee reached for the cutters in their pocket. The open pair of cutters penetrated through the employees leather gloves causing a small laceration to the palm of the right hand.

Employee was sprayed in the face with oil air while repairing air dryer.

During routine annual physical employee was found to have a shift in their hearing that was confirmed by the specialist to be partially occupational related.

Employee was shoveling spillage when they started having muscle cramps, felt hot, weak and shaky.
