Employee states that he injured his back while loading materials.
EE was walking through tool box storage area when he tripped on the corner of a tool box and fell into another tool box contacting his face. The employee received a fracture to the orbital area of the left side of the face.
OPERATOR PARKED HIS TRUCK AND PLACED TIRE CHALKS UNDER TIRES, STEPPED BACK TO GO BACK INTO HIS TRUCK AND STEPPED ONTO THE CHALK AND TWISTED HIS LEFT ANKLE.
The injured employee climbed a concrete form wall to retrieve a piece of plywood which had come loose from another form when he missed a tie-off point with his personal fall arrest equipment resulting in a fall from the 9 foot elevation. The employee sustained head and neck injuries as a result.
After our investigation, it has been determined that the employee injured her knee while walking down a flight of stairs by the #16 ball mill.
Two employees were trying to move a hoist brake into the shovel house. They maneuvered it up an 8"" step and in a horizontal position. As they tried to lay it down to roll it in, the brake slipped and landed on the employee's hand. The brake weighs 1,260 lbs.
After our medical investigation was completed on 09/13/2013, due to the employee previously having a fractured nonunion of his left clavicle (personal), it has been determined that the employee had a worsening condition along with the symptoms, after he fell.
A feed plate slid down and contacted a Crush & Convey Maintenance employee's right foot.
Employee felt pain in back after hitting a berm on the haul road, reported incident on 4/18/2013.

Employee was venting a railcar. Once the pressure was released the employee walked off the railcar on to the trans loader and down the stairs. He walked 50 feet and reported to the other employees he could not breathe.
Employee twisted his body around to retrieve a tool from his tool belt when he states he felt pain in the back. The employee received days away from work.
Maintenance employee smashed finger between a valve and a pipe.
Employee stated that while trying to reposition a piece of channel iron he dropped on his foot causing a fracture to 3 bones in his right foot.
EE was reaching out to grab a piece of grating with a grating hook while holding on to a handrail with his left hand. His foot slipped allowing him to fall forward extending his arm until the left shoulder dislocated.
Employee was working as a carpenter and mis stepped and sprained ankle.
Contractor employee working as a carpenter miss stepped and twisted his ankle. The injury resulted in a fractured ankle.
EE was walking to the bus after completing his shift when he stepped on a rock and rolled the left ankle and twisting the right knee. The employee missed his next scheduled shift and was then evaluated by a doctor and given three additional days off of work.
Nightshift on 09/14/2013, employee was operating the 562 haul truck when the 591 haul truck, made contact with his haul truck.
Operator of the 508 haul truck states that she hit a pot hole.
The operator of the #4 water truck was climbing back onto the first rung when her foot slipped and her right knee popped.
The rubber tire operator was hit with a piece of fly rock from a blown haul truck tire.
Contractor employee received a serious contusion resulting in surgical intervention to correct a compartment syndrome to the left forearm. The injured employee's arm was caught between a steel cross brace and previous installed horizontal beam while attempting to connect the cross brace.
Contractor employee bent over at the waist to pick up 2 2X6X16 pieces of lumber when pain and discomfort was felt in the right lower back region.
Crush & Convey helper tripped over some dirt in front of Metcalf and fell forward onto his left shoulder.
Employee was walking and the grating tipped resulting in the employee falling to the ground. Initial medical evaluation resulted in the employee returning to full capacity work. During a routine occupational medicine follow up on 4/17/2013, the employee stated that he was still in pain and medical evaluation revealed the employee will be out of work to allow time to heal.
After our investigation was completed on 3/28/2013, it was determined that a Crush and Convey employee after he slipped and fell, using his left hand to catch himself, injured his left shoulder.
Employee was releasing a hook from the strongback, when the weight shifted and the hook smashed his left index finger.
Employee was carrying an electrical panel while walking backwards and fell. Employee reported incident on 12/31/2013.
A mechanic was changing out drill steel when the bit sub tightened the deck table swung and the employee's left foot was caught between the deck cable and mass brace.
Employee stepped on a pile of dirt that gave way causing his ankle to twist.
Employee states that he felt shoulder pain after performing maintenance on 12/18/2013, did not report injury until 1/15/2014.
Operator of the #7 water truck hit a pothole leaving the water stand causing a low back pain.
Employee felt pain in lower back in December 2013, did not report until March 2014. Medical evaluation on 4/3/2014 placed the employee on restricted work activities.
Employees were repositioning forks on forklift when a stuck fork released and caught the employee's middle finger between the frame and the fork. The employee pulled their finger from the pinch point and fractured their finger tip as well as avulsed a large section of flesh from the finger tip.
Employee was sitting in a chair when he fell out of the chair and felt pain in his right wrist.
Employee was standing on a dozer frame that had been removed from the dozer, blade was laying front down with arms vertical. When she stepped off the top of the frame onto a hoist pivot cylinder link. The link shifted causing her loss of balance and fell to the ground approximately 42 inches. She landed on her left arm resulting in a distal radial fracture.
EE walked to a pallet to retrieve a small piece of angle iron and as he bent over to pick it up he lost his balance falling into the pallet. The employee put his hands out and the right hand went between the slats on the pallet catching his thumb and forcing it backwards. Surgical intervention required.
A carpenter using a 1/2"" drill with a 3/4"" paddle bit was drilling a 2x4. The drill caught a nail, causing the handle to twist. The 4th and 5th finger on his right hand was caught in the twisting motion of the handle, bending them away from the other fingers. Employee has a fracture of the 4th Metacarpal bone.
Employee was riding in a man van when the van hit a pot hole causing the employee to experience pain in his neck.
EE was assisting work crew in the installation of a conveyor table by way of rigging when the load shift and struck the employee in the shoulder. This caused ligament separation in the shoulder.
Employee was operating the 511 haul truck when he hit a bump in the haul road.
An employee lost his balance while stepping backwards to allow another employee access to a walk way. The employee tripped and fell backwards, landing on his shoulder fracturing his left shoulder blade.
Employee was walking down the ramp when she slipped on loose gravel causing her twist her knee.
Employee smashed left ring finger when the jack stand collar slid down at he stood the jack up.

The operator of the 602 haul truck was traveling up the Northwest Ramp, when she made contact with a high spot in the haul road, causing her to injure her lower back.
Employee was walking to the portable restroom when he slipped on a piece of grating placed in front of the restroom to clean off boots. As he slipped and fell to the ground he placed his hand out to catch himself and struck the grating causing a laceration to the finger requiring sutures.
Employee was injured when the 30-O van hit a pothole.
The employee was riding in the 102-O man van (back seat), when the man van struck some pot holes, causing the employee to be jarred. The man van operator was traveling at approximately 5 mph.
EE removed a scaffold leg from a scaffold during dismantle when incorrect positioning allowed to much force to be applied to the left knee causing a sprain. The employee was trying to hand off the scaffold leg to another employee when the incident occurred.
Employee went to throw a hoe under a conveyor and hit his index finger on the conveyor structure, fracturing it.
Employee was traveling down the steps from the ball mill and missed the last step, causing her to fall and cut her left knee and twist her right ankle.
Employee states that while operating the 584 haul truck that she was rough loaded at the 44 shovel.
EE attempted to take a step to begin walking and stepped on a rock forcing her knee backwards.
The operator of the 646 haul truck stepped into the parking ditch and when he planted his left foot, his left knee popped.

A machinist smashed his right hand between a motor grader cutting edge and the mold board, causing fractures to the middle and ring finger.

Employee was stepping out of van when she stepped on a water bottle and fell down at which time she heard a pop on her left ankle.
Employee was assigned to fuse 4 inch pipe. Employee reported on 04/11/2013, that she had injured her hand, bur refused medical treatment. She was brought to the clinic on 05/01/2013, released to full duty 05/14/2013. Employee was still having pain in her wrist and returned to the clinic on 08/07/2013, given restrictions.
Employee was operating the 57 Loader when he felt a pop in his right elbow while operating the bucket lever.
The 590 haul truck was injured being loaded at the 17 shovel on his night shift of 01/22/2013.
Employee was cutting Q-decking (sheet metal) with a Metabo cutting tool and cutting disc. The tool ""kicked-back"" and the employee lost his grip. The cutting disc contacted his left index finger causing a 1 inch laceration. The employee was treated at the Morenci Urgent Care facility, received 6 sutures and released to return to work without restrictions.
Carpenter was stripping forms and bumped his head on a support member.
EE received a laceration to the right thumb when it was caught between a railroad tie and a pipe support. A coworker mishandled the railroad tie and dropped it onto the employees hand as he was setting it into place.
EE received a laceration to the right pinky finger while making an adjustment to the retaining ring of a LeJune gun. Three sutures were required to close the wound and the employee returned to full duty.
Employee felt something crawling on his back, he attempted to swat it away when it fell into his pants and he felt a sting to his buttocks.
The employee was using a hand held grinder. When he finished his task. He lifted his face shield and looked up. It was at this time that he felt a foreign body enter his right eye. The eye was flushed and he was taken to the on-site clinic where tweezers were used to remove the foreign body.
Carpenter employee received a laceration to the palm of his left hand after reaching out to grab a section of rebar to steady his balance. The rebar came into the back side of the gloved hand and requires three sutures to close the wound.
Employee was trying to open a gate while driving a forklift when the gate wouldn't open. Employee's hand was made contact with the forklift rack causing a laceration to the hand.
Employee pinched finger between a ratchet and square tubing.
Employee was striping wire with a razor knife and while cutting the wire the knife slipped and cut through his leather glove. The employee received a laceration to the left thumb requiring six sutures to close the wound.
EE was attempting to relocate a battery powered potable bandsaw when he mishandled the tool and operated the trigger while attempting to grab the tool with the other hand. This caused him to cut his left middle finger which required sutures to close.
Pipefitter helper was removing the hoisting ring of a three legged bridle; the rigging was transported to site on the forks of a loader. Ring became wedged on a fork during transport. Employee positioned both hands on each side of the ring to pull it free, ring twisted and pinched the tip of the 5th finger between fork and ring. The finger tip was diagnosed as a tuft fracture.
Injured employee was attempting to strike a bull pin with an 8 lb sledge hammer to assist in the connection of structural steel when his left index finger struck the edge of the flange on the steel beam causing a laceration.
EE received a foreign body to the eye during shift on 10-2-13 but did not realize. The following morning the employee returned to work and the foreign body was removed and prescription given.
EE's foot slipped from a form wall as the employee attempted to step off the wall after descending the wall with fall protection. As the employees foot slipped EE fell 2 feet to grade landing on the left side of the body causing contusions to the knee. A fractured rib was discovered upon follow up on 10/25/2013.
Employee #1 was grinding panels inside the Fine Ore Bins at the concentrator. A second employee was grinding next to employee #1 and hit a rough area on the panel causing the grinder to jump to the left side striking employee #1 on his right elbow. The employee sustained a small laceration requiring 3 stitches. The employee was treated at the clinic and released back to work.
EE received a loose foreign body (dust) to the eye. Eye was flushed and debris removed. Several days later the employee reported discomfort and an abrasion was discovered. A prescription medication was provided for the right eye.
Employee sttempting to remove a hitch one service truck utilizing a hydraulic ram to push it off due to hitch being slightly bent and would not come out. A small piece of metal broke off, going under face shield and making contact with employee lower left cheek, requiring 7 stiches.
Employee cut right index finger with a utility knife, while skiving the belt to install belt clips.
Employee was attempting to access an elevated work area when he grabbed a horizontal scaffold runner with his right hand and the outside of his right forearm was contacting the vertical upright. When the employee pulled up lateral force was applied to his forearm and it caused a hairline fracture to the ulna.
Employees were stripping plywood forms; an employee had removed a 2'x 1'piece of plywood. When moving the plywood a corner struck EE above his safety glasses and below his hardhat brim, causing a laceration requiring 5 stitches
Employee pinch left index finger between a sling and a crane hook.
Injured employee received a foreign body to the eye while walking by another work area. The object was removed and prescription medication given.
While removing the u-joints from a drive line with a hammer and punch, Employee had the hammer slip from his hand striking the thumb on his left hand. Employee was transported to medical facility and received three stitches on injured thumb band-aide and was returned to work.
EE received a laceration to the left index finger while assisting in the installation of a steel cross brace. The injured employees finger was caught between the line up pin and the steel as the pin was removed. Sutures were required to close the wound.
Injured employee attempted to drive a form stake into the ground while using a 10 lb sledge hammer and missed the stake with the head of the hammer and stiking his right index finger against the stake and the handle of the hammer causing a laceration.
EE was using a 6"" angle grinder to cut a lifting eye from a steel beam when the cutting disc of the grinder broke causing the EE to lose control of the grinder. The grinder fell back onto the employees left arm causing lacerations to the arm.
Laborer was sprayed with solution containing lime, slurry and water while he was opening the #22 ball mill classifier box lower cam gate.
Employee was removing the C unit fine chute hatch cover, as he loosened the bolts the cover fell, striking the back of the employee's hand.
Crush & Convey lube crew employee was removing a guard to service the A2 secondary screen in the Metcalf Secondary crusher, when the guard became stuck and during removal the guard cut his left hand.
Mechanic cut his left wrist while removing a water pump off of the 601 haul truck.
Employee was attempting to start an air compressor when he lost his balance and fell out of the bed of the truck hitting his head.
Employee was repairing hose when his folding knife closed causing a laceration to the right finger.
Employee struck self in the forehead with a socket that was on the end of an impact wrench while he was removing cutting edges off of the 27 rubber tire.
The employee struck the inside of his foot with a 16 lb sledge hammer. The force of the impact caused the outside of his foot to come into contact with sharp metal that cut through his boot and lacerated the outside of his left foot. The wound required sutures.
Contractor employee received a laceration to the left thumb when it was caught between the flange of a steel duct and a flatbed trailer during the off loading process. A forklift was used to offload the duct and the duct had slid off of the tip of the forks while employee had his hand on the load. Employee received sutures to close the wound.
Employee was holding and running an impact on the bottom bolt and another employee was tapping the top bolt with a 4# hammer. The employee missed the top bolt and hit the other employee in mouth with the hammer.
The 41 shovel was moving to align track pin hole, a rock projected and struck the employee on his lower right chin/neck. The employee was approximately 20 feet away from the shovel.
Employee smashed her hand between a copper cathode bundle and bander. It is possible that she has a possible fracture of the left ring finger.
Employee was working in the R8 R9 chute using a jackhammer when he lost his balance and struck his left little finger between the jackhammer handler and a block liner resulting in injury.
Employee complained of hand swelling and redness on left hand ring finger. Initially thought was due to a presumed insect bite.
A Haulage employee became sick while on the ground waiting on the mechanic to work on her haul truck.
Employee was working on the 49R drill when he was overcome with a heat related illness.
