Accident type, without injuries

Carrier had 2 technicians enter the Crusher 3 cargo elevator, elevator stop in between floors, crusher 3 control room called electicians, they reset elevator both carrier technicians were fine no injuries or medical assistance needed, they were in elevator approximately 25-30 minutes.

It is likely a hot idler bearing caused the conveyor belt to catch fire. MSHA Accident Hotline was called at 1:38pm to report the event. There was not anyone in the area at the time of the fire.

Employee directing lift operator. The forks needed to be moved together. The slide bar has pinch points. When bar moved it allows the forks around the hook. The bar did not slide, he placed one hand above the heel of the fork and one hand on attachment loop where the fork attaches to the bar. fork pushed. Fork swung back catching fingers between the fork and the outside hook

On 11/18/2015, an employee was walking up the stairs on the extension rougher side when she felt pain in her right ankle. She was evaluated at the clinic and released. On 11/20/2015, the employee returned to the clinic and was given restrictions.

A leaching employee lost his footing and hurt his ankle. The employee was helping push a 4"" pipe causing the him to roll his ankle.

On 9/20/2015, an employee was getting into a van when felt pain in his hip. He did not report the event until the next shift and was taken to the clinic.

The employee was tasked with whipping drip lines when the right line and single became disconnected from the 4 inch feeder. The employee, still holding the left drip line and her hand slipped causing her to fall off balance. The employee fell to the left side and landed with the left leg underneath her body. The incident resulted in a spiral fracture to the fibula.

On 8/7/15 an employee was in the process of connecting an airline for a pump and stepped into liquid lime. He notified the fill-in supervisor and Sr. Supervisor but refused medical attention. Both Supervisors advised the employee to rinse off his foot and apply Neosporin. On 8/12/15 and was treated for an infection resulting from chemical burns to the right foot.

Employee was dismounting the HV023 Service Truck when he missed the bottom step and fell to the ground, landing on his back.

An employee was attempting to remove a radiator cap when he was splashed with hot radiator fluid on the hand.

Employee received raffinate/sulfate in left eye causing Alkaline burn to his cornea and conjunctive sac.

The operator of the 571 Haul Truck backed into the dipper of the 40 Shovel causing pain in his lower back.

An employee stepped in a tire rut and felt a pop in his knee.

Employee was installing liners on the HRC 3000 at the Metcalf, liner was attached to the lifting apparatus (magnet) outside the mill by another employee and was guided into the mill, where IE was to place and secure slings for placement of liner, upon placing slings, magnet released liner striking EE's right foot, fracturing 4 metatarsal bones on right foot.

Employee was traveling in a man van and hit a pot hole and felt pain in back.

Employees were lifting a frame into the bed of a truck when one employee tripped on a rock and felt pain in his lower back.

Employee was using a bottle cart to move an Argon bottle to his work area. As the employee was going over a 3/4"" air hose, the cart shifted causing the employees right pinky finger to be caught between the cart handle and dipper bell. The employee suffered an open fracture requiring 6 sutures.

An employee was pinned between the wishbone and the rear differential of the 793 Haul Truck causing bilateral rib fractures.

Employee lost footing on top of molten sulfur trailer after loading process was complete, causing employee's left leg to fall into manway coming into contact with molten sulfur in the trailer. Employee received second degree thermal burns to the ankle. No sulfur came in direct contact with the skin.

Driver thought was finished unloading sulfuric acid. Driver disconnected hose before completing offload and was sprayed with acid.

An employee was assisting a co-worker using an air hose & lance when the hose separated from the lance causing the hose to whip around striking the employee's hand, resulting in a fracture to the right thumb.

An employee developed minor chemical burns to both ankles after stepping in concentrate.

Individual was walking along same level surface and slipped, in the process of attempting to regain balance employee sustained left wrist fracture.

On 3/27/2015, an employee felt pain in his shoulder while performing installing a filter. The employee was evaluated at the clinic at returned to work to his normal position. After continual pain the employee was referred for an MRI with positive findings for a torn rotator cuff and sent for surgery on 6/17/2015. Safety department was notified on 8/11/2015.

Driver inadvertently pressurized a hose he was disconnecting by bumping open the 1/2"" air supply ball valve. He was sprayed with sulfuric acid from the hose upon disconnecting the air line. A palm sized area on his thigh was burned when the acid passed through an opening in his protective suit.

An employee was replacing the drill steel when the breakout wrench brushed his hip causing him to lose his balance and he contacted his forehead against the handrail causing a laceration.

Employee was replacing scaffold plank in the storage rack. He went to lay the scaffold plank down and the plank slipped out of his grasp and landed on the employee's hand pinching his right index finger between it and another plank on the stack

An employee was waiting to be loaded when a rock rolled and struck the rear tire of the truck causing the operator to experience pain in his lower back.

An employee was walking when he stepped on a rock and twisted his right ankle.

Employee was replacing liners at the 801 feeder, while passing the liner into the feeder opening, the liner was released before the employee had control causing liner to drop smashing left ring finger. Resulting in chipped bone on left ring finger.

An employee slipped on the last step outside the control room of the Metcalf Concentrator. The employee stated that his right hand was on the hand rail and he was holding a 6"" sling in his left hand and making his was down the stairs. The employee also stated he felt a pulling sensation in his left leg as he was truing to balance himself when he slipped.

On 9/29/2015, an employee was replacing guards on the 4c conveyor. The following day, 9/30/2015, the employee reported pain in his right shoulder and radiating towards his elbow.

An employee was walking to her truck and tripped and fell, landing on her knee causing a minor bruise.

An employee was pulling drip lines to be installed, when he inadvertently stepped on an unstable section of material and allegedly hyper-extended his right knee. Employee was taken off site for medical evaluation and was put on restricted duty.

The employee was assisting in pulling out a roll of geosynthetic material. It is done by attaching a vise grip clap to the material and pulling. As he started to pull the clamp slipped off and he fell backwards into an anchor trench hurting his back.

An employee descended an 8ft ladder, once completely on the ground he turned to close the ladder and felt a sharp pain in his right knee.

Employee was tasked with cleaning the floor and maintaining housekeeping when he rolled his ankle and fell on his knee.

EE was descending down a set of outdoor stairs. He did not get his foot fully placed on one of the steps and twisted his ankle.

The operator of the 17 shovel was coming down the stairway at shift change when EE lost footing causing EE to stumble down the stairs and injure right wrist.

An employee believes he possibly struck his hand against a jumper frame while walking by. The employee felt pain in his hand and removed his glove and he noticed a small laceration on his knuckle and swelling of the hand.

On 7/3/2015, an employee went to walk down the catwalk his foot became stuck in the grating and rolled his ankle. On 7/6/2015, the employee returned to the clinic and was given restrictions.

Uneven rocky terrain. Employee has been evaluated several times by different physicians at occ med and released to work fit for full duty with no restrictions. On the last evaluation employee was put on restriction/light duty.

The operator of the 515 Haul Truck was backing up to the 46 Shovel to be loaded. The Haul Truck operator did not follow the shovel operator's directive when he honked his horn for the truck to stop. The haul truck operator continued to back up until the truck hit the dipper.

Employee was carrying a 60lb roller and had a set of steps (2 steps) to step up and when he stepped he felt a strain in his lower back.

On 5/28/2015 an employee was riding in a man van to her truck assignment when the van hit a pothole causing the employee to feel pain in her neck. On 5/29/2015, the employee attended a follow-up appointment and was given restrictions limiting her ability to perform her normal job.

On 4/28/2015, an employee received a bruise on her lower arm when she struck it against the boarding ladder. On 5/15/2015, the employee returned to the clinic for unresolved pain and was given restrictions.

On 4/27/2015, an employee removed a rock that was wedged between the push arm and tilt cylinder on a crawler at which time he was evaluated and returned to work without restrictions. On 5/14/2015, the employee returned for medical evaluation due to unresolved pain in his shoulder, and was given restrictions.

Technician informed me last rotation that he had back pain but was not sure when it happened. We decided to let him rest to see if it got better. He still has back pain. The technician has given different dates and delayed in fully reporting this. We are investigating the validity of this claim.

An employee was attempting to change the steering cylinder when it slipped off the frame and pinching his left hand between the cylinder and the jack plate.

Employee was working on truck 541 replacing electrical harness. Employee was routing a harness and noticed that there was slack in it. Employee pulled on harness and felt a pop in his left shoulder. Employee reported incident to the fill in supervisor.

An employee was replacing liners when used a pry-bar to attempt to free a stuck one. The liner broke free and struck the employee's finger causing a laceration to the left middle finger.

On 10/6/2015, employees were using a cutting wheel to remove a bracket. The next morning, 10/7/2015, the employee came into work and felt discomfort in his eye. Upon evaluation at the clinic it was determined the employee had a foreign body embedded in his eye.

EE was walking into the break room when he tripped and chipped his tooth on the chair.

On 7/29/2015, an employee had slag contact his left ear, at which time no treatment was given. On 8/3/2015, the employee returned to the clinic for a follow-up evaluation and was treated with OTC ointment for a minor infection. On 8/7/2015, the employee was given an antibiotic to treat the infection.

AN EMPLOYEE WAS REMOVING A ROCK FROM A DRILL HOLE WHEN THE ROCK ROLLED CAUSING A LACERATION TO THE RIGHT INDEX FINGER.

Was assigned to reseal left suspension cap on 571. Removed bolts and attached tooling cap then started to raise cap, but it was stuck. Tech instructed forklift driver to move forward. Cap suddenly became free and smashed his finger between the cap and frame.

Employee was completing task of prepping 30"" stainless steel pipe with a grinder, when employee set the grinder on the plywood without bringing the Tiger paw disk to a stop. EE's right upper part of his thumb came into contact with the Tiger paw disk causing a laceration.

The employee was cutting the box open from right to left, towards her. The utility knife went through the tape on the box and contacted her left lower arm causing a laceration.

Assigned to complete a post oil change on 508. Connected the evac pump and found it did not work. Went to replace evac pump, removed the air hose thinking that all of pressure was released but there was still pressure in the oil hose. Employee disconnected the oil line to the pump, it came off suddenly and hit his left index finger, causing a laceration that required stitches.

Employee was grinding in a tight area, he released trigger with the left hand and the grinder wheel caught in the area that he was grinding. The grinder then struck his right hand causing a laceration to the right hand.

On 2/23/2015 an employee stated that the dipper of the 41 Shovel struck the headboard of his truck. Reported on 2/24/2015. On 2/24/2015, the employee stated he was rough loaded at the 40 Shovel.

On 8/5/2015 an employee was riding in a man van with BO AC when she began to feel sick. She was taken to the clinic and given fluids and released from care. On 8/11/2015, the employee was by her private care physician and given a work exemption for exposure to heat.

An employee began to feel heat related symptoms on 6/19/2015 at approximately 10:30am. He was evaluated at the clinic and transported to the hospital for higher level of care.

On 10/1/2015 at approximately 21:30, an employee was discovered lying on the ground suffering from cardiac arrest. Emergency Personnel were dispatched, and began CPR. Employee was taken to Gila Health Resources.
