Employee was standing next to the loader straighten out the rock dust to rock dust at the end of the shift, when a piece of the rib rolled out from the top of the binder striking EE in the right ankle. Employee was treated with a fracture to EE's right ankle.

Employee stated, EE was walking by the corner in number nine entry, when the corner fell out striking EE on the left side of EE's upper back.

While loading in the number ten entry, a piece of coal rib rolled out striking the employee's left ankle.

Employee was injured when EE's left foot was struck by a rock that dislodged from the mine rib. EE was diagnosed with a fractured metatarsal and EE's foot was temporarily immobilized.

While mining in #3 entry of section. Shuttle car operator was struck by rib burst to left side after being loaded by miner returning to feeder. Causing several small scratches and cuts to arms and side of face. Employee is going to follow up with a x ray in a few days to make sure no fractures.

Just after loading shuttle car, miner operator was struck by rib, causing them to fall injuring their left foot. Had x ray done at hospital, showing no fracture, but going to x ray again after swelling goes down to make sure.

Rocks fell off a highwall striking rock truck. The employee suffered a contusion on their left upper thigh. Received medical reports to make the determination on 3/2/2026.

Approximately 30 kg rock released from the face near the springline in active underground workings. Rock fell 5-8 feet and impacted the miner's arm against the workbasket handrail. Rock was partially restrained behind overlapping face mesh. Miner sustained arm injury.

Employee was installing a roof bolt just inby spad no. 6978 on 1A section, when a rib rolled striking EE in the right lower leg, fracturing EE's Fibula.

Employee was walking to miner when piece of rib fell onto EE's foot. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

An employee was loading a face with explosives when a rock fell from the face and caused a laceration to the employee's leg. The employee received 7 stitches to the leg laceration at the Hospital.

Injured employee was operating a shuttle car when a small piece of coal fell from the rib and into the operator's cab and struck their left little finger. Employee suffered a small abrasion/contusion to the finger. Employee worked their next shift but sought treatment the following shift. Employee began missing work on 9-12-25 and missed 2 shifts of work.

IE was looking in the dust box when a rib rolled out and hit them into the bolter.

Shuttle car driver was entering EE's car when a piece of coal rib struck EE in the back and the right ankle. Started missing work on 7/25/2025 for inflammation.

A piece of loose material contacted the employee on the right arm and left leg. Required stitches.

Employee was unloading a beam from the scoop. When they went to set the beam down, the rib rolled and large pieces of material struck them in the back, neck and back of their head.

In St Leo Mains walking from loader to miner after getting a water bottle at #1-#0 crosscut 109 block the rib "popped" dislodging a softball sized projectile that struck them in the left side of the face resulting in contusions to left eyebrow and nose. No problems with equipment or compliance issues, training, or PPE.

Employee was hanging line curtain on the #2 section in the #4 entry when a piece of coal rib gave loose striking them in the lower back and ankle.

IE was changing shearer bits on the Tailgate drum while kneeling on the ground. The rib sloughed off striking the employee knocking EE to the ground. IE was struck on left side of EE's body causing a bruise and when employee tried to stop EE's fall EE was cut under right armpit causing 6 sutures and fractured EE's right wrist.

EE walked beside the bolter in the bolted entry to throw cable out of the way.A rock fell from the rib 8' high measuring 16" long by 2.5" wide.The rock hit the EE on the left forearm then the left boot.When it struck EEs boot, the rock went through the top of the LaCross Metatarsal guard mining boot causing a laceration to the top of EEs foot around the toes.EE got 2 pins/sutures.

when shoveling in the beltline the rib rolled down onto their left leg resulting in bruising. Employee was transported to the hospital for precautions but was released and the doctor said no work for a week to rest their leg.

Employee was in the #4 entry when a coal rib gave loose striking them on the right ankle. Employee finished the shift and sought medical attention on 6/12/25 for a fracture ankle.

Employee was helping set up Highwall Miner when they walked around front left track, when a mud ball fell from highwall and busted on ground throwing debris, hitting employee in groin area.

A rock fell down the shaft and struck the employee working in the shaft on the back of the neck. A contusion was incurred to the impact site at the back of the neck.

EE was turning S/C into the right side dump. EE misjudged the turn and hit the corner of a coal rib with the canopy, which resulted in a piece of coal dislodging and coming in contact with the EE, striking the EE on the right ankle, resulting in a fracture.

Employee was standing by the continuous miner when section of rib came off striking EE's left foot against the continuous miner.

I.E. stated they were on the clean-up car when a piece of the coal rib slid out from behind the rib wire pinning their leg against a shuttle car. X-rays showed their Fibula was broken.

While putting a crib block under the continuous miner, the corner rolled striking the employee pushing their face into the side of the continuous miner causing swelling around the left eye. There was no treatment provided at initial doctor visit but they returned to the doctor on Monday 7/21/25 and was treated with prescription eye drops. We were notified of treatment on 7/28/25.

I. E. stated that they were standing next to the rib and a o=piece rolled out striking employee in the left ankle. Employee went to hospital x-ray showed nothing broke they followed op with Ortho. doctor that took them off work due to muscle injury. Employee was issued crutches

Employee stated employee was bolting off the miner on 78MS when a piece of rib rolled out and struck the employee on top of the foot. Employee sought medical attention and was diagnosed with a fractured foot.

EE was struck by a rib roll in the right foot while setting over in cut with EE's continuous miner. EE did not initially seek medical treatment for EE's injury. EE reported a sharp pain on 5/17/2025 and sought medical treatment at that time. EE was diagnosed with an Achilles Tendon rupture on 5/28/2025 and placed off work at that time.

Two miners were setting up to load a round. They were scaling down hazards on the face. The IE then approached the face to inspect the holes in the burn. IE turned to talk to the other miner and was struck by a falling rock. IE was transported to the hospital, released, and referred to a specialist. IE saw the specialist and was scheduled for surgery.

While drilling a piece of the mine rib measuring 12x12 fell from above the work area striking the employee in the back of the head.

The continuous miner was parked next to a rib and EE was checking something on the machine while the belts were down. The rib made a little noise and came down. EE didn't have time to move and the rib pushed EE's knee into the miner bruising and cutting the knee.

Employee was measuring the area to cut and set posts when the rib rolled out and struck EE's leg resulting in a bruise leg. This resulted in a lost time accident.

Miner operator was starting to load a car when the face fell in leaving a pile of rock just inby the last row of bolts. A rock then fell out just inby the last row of bolts landing on the pile of rock causing the rock to slide in on the operator's left ankle. Resulting in a fracture the miner operator started missing work 1/24/2025.

Employee stated that EE had backed the miner out to set over in the cut when a section of the rib rolled out and struck EE on the right hip/back and leg. Employee was treated and released by a doctor and continued to work until 2/24/2025 when EE went back to EE's doctor and was taken off work.

Employee was rib pinning off miner when a piece of rib slough age struck the rib pinner then struck employee's lower leg. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was struck by a coal rib when working on the number 8 drive.

Employee was waiting for the bolt crew the finish spotting a bolt when they were struck in the back by a rock from the rib.

The employee was walking through a crosscut. EE kneeled down when a piece of rib rolled off striking EE on EE's lower left leg. This resulted in a broken left tibia and fibula just above the left ankle.

Employee was in the #2 entry when a piece of loose coal rib came loose and struck EE in the upper back. Injury was not the result of a training or compliance issue.

EE was working on the #1 West Section (2) breaks outby the feeder loading crib blocks on the scoop. While loading the crib blocks, the corner rib rolled out measuring 5'x 3'x 1x1/2' thick striking the employee on the right foot resulting in (5) broken bones.

Employee was injured when their right leg and foot were struck by a rock that dislodged from the rib. Employee's foot was temporarily placed in a walking boot.

992K Cat loader - loading shot material into 777F Rock Trucks in coal pit. Employee going into pit to get a bucket of shot material. Right side of highwall fell and struck front of loader and front cab area.

Employee was operating the continuous miner in 7 South mains. A piece of rib fell out while mining between 4-5 entry at 10.5 X-cut and struck the employee pushing employee to the ground. The employee suffered 2 fractured ribs as a result of the incident.

While rib bolting off of the continuous miner in 7 South mains between 4 and 5 entry at 10.5 xcut, a piece of the rib fell from above the rib bolt striking the employee on the right leg causing a fracture.

While setting in the shuttle car at the feeder the corner of the rib rolled off and some of the coal fell into the cab pinching the skin on the back of the right arm causing two small lacerations. 3 sutures in one and 5 sutures in the other.

EE was loading supplies on a roof bolting machine, when a piece of rock fell from the rib (44inches long, 8 inches wide and 6 inches thick) striking EE on the right leg resulting in a laceration and 3 fractures to the lower leg (below EE's knee).

IE was installing rip pins in the CM section when the rib rolled striking the employee. IE suffered fracture to EE's hip and pelvis area.
