Operator was dismounting D-6 dozer when EE's foot slipped from the blade arm due to wet and muddy conditions causing EE to hit EE's left knee on the arm. Operator had just returned to work from a double knee replacement and the impact on the knee caused the incision/scar area to reopen.

Mostrando 50 de 46,436Employee was on a safe rack when the flooring gave way. Employee did not fall all the way through and fall protection was in use. During the process of catching self the employee bruised the left arm and left leg.
The employee met with supervisors in a parking lot on the east side of the building. After the conversation, EE turned to walk back to the office and tripped over a wheel chock (angle iron) because EE did not fully clear it. EE fell onto EE's right knee and elbow. EE was wearing steel-toe boots, and no other PPE was involved.

Employee was walking into the maintenance office when EE tripped, fell and land on the maintenance lunchroom glass door. Employees elbow/arm broke the glass on the door resulting in laceration of the tricep area.

Employee was loading supplies into EE's scoop. The employee's foot got caught and EE fell. As EE was falling, EE reached EE's arm out and fell onto a pallet. A nail from the pallet poked EE in the right forearm requiring one stich to close the wound.
A miner tweaked EE's knee while accessing the boat to work on the water pump. The injury occurred on 5/15/2026 and was placed on restricted duty on 5/18/2026. The miner has returned to full duty on 5/22/2026 with no lost work days.
Emloyee stepped out of a medium duty vehicle onto uneven ground and rolled their ankle. This caused a fracture to the distal fubula.
Employee was on the lowest step of a 4 foot step ladder to work on a F250 and stepped down twisting EE's right ankle on level ground causing a sprained ankle requiring a day of rest and no climbing ladders for 2 weeks.

Employee was in the process of removing a hydraulic hand pump and jack from the top of the tub base and hung EE's foot on a hose and fell forward and EE's thumb on the left hand was caught between pump and ram which caused a fracture of the first joint of the left thumb.

Ee was stepping out of conex holding a box of uniforms and EE rolled EE's right foot on the lift gate. Seen at clinic, diagnosed with right ankle strain, and released with restrictions that do not preclude EE's full duty as a planner.

A miner was working from a ladder while applying a patch to a screener feed box. While reaching, the ladder slid, causing the miner to fall onto the ladder. The miner sustained a laceration to the right knee and a fracture of the right patella. The miner was prescribed medication and lost time.

Operatpor was fueling truck and tripped on fuel hose while replacing the hose in holder.
While exiting a water truck, the employee missed the second rung from the top of the ladder and fell approximately three feet to the ground. During the fall, the employee attempted to regain balance by grabbing the door, which caused EE's body to shift to the right. Upon impact, EE landed on EE's right elbow and reported pain.
Employee was climbing down the steps of a blast hole drill, when the employees first foot touched the ground EE let go of the handholds with EE's hands, at this moment the other foot got caught on the last rung of the steps causing EE to fall backward onto the ground. The employee had soreness and stiffness in the lower right back.
IE slipped while descending some steps in the prep plant. IE did not maintain three points of contact while on the stairs. Injury was not the result of a training or compliance issue.
Employee was shoveling on A1 header bulkhead when EE stated EE slipped on a small hill before the belt line and went to catch self and when EE went to catch self EE caught EE's hand in-between the top roller and bracket.

IE was running string line for a new refuse drive and when EE turned around, EE tripped on another section of the line that had been run. When IE fell EE hurt EE's foot.
