CONTUSION OF RIGHT FORN WALL OF THORAX, CONTUSION OR RIGHT SHOULDER, SPRAIN OF LIGAMENTS OF LUMBAR SPINE

I.E. was carrying an air tank to the face when EE slipped on shield #2 and the tank fell on EE's finger.

Subject was exiting a work pick up truck when EE twisted EE's left knee. Subject continued to work and went to doctor on 7/14/2026 and was given clearance to work but with restrictions.

Employees were removing jacks from under #2 kiln tire (jacks free of any weight). Employee was guiding jack off the base; it came off awkwardly and pushed EE off balance. EE placed EE's right arm out to catch myself and felt a pop in EE's shoulder.

Employee was walking up to the continuous miner, as they approached the miner they slipped and fell striking the right side of their face on the miner resulting in fracture to cheek bone.

During a pre-trip inspection, the rigger was directed to the opposite side of the engine compartment. Instead of climbing down to go around, they walked across stored wood mats on the crane. The mats shifted, causing them to fall and fracture their ankle

Coworker was on uneven surface, mis stepped, lost their balance and fell. During the fall, EE hurt EE's wrist.

As employee was climbing into their truck at the mine, their hand slipped off the handhold and they fell to the ground. Employee sustained a strain to their right hip and back.

Miner was going down a set of stairs and tripped. When EE grabbed the handrail to catch self EE's finger caught a sharp edge and was cut. EE needed 8 stitches.

Employee descended a crane ladder and missed their grip causing them to fall onto their back. Event went reportable 7/8/2026.

EE was walking down the steps of the surge pile tunnel. missed last steps, caught themselves with the handrails, tightened elbows and right shoulder, popped elbow, and now has pain in right shoulder to right elbow. EE went to clinic and was given restrictions.

Employee stepped off of excavator on uneven ground and sprained knee

Injured EE was was walking near the Mill 5 Feed box and slipped on some mud lost balance and reached for the handrail to catch themselves. While doing so they felt a pop in right shoulder.

Employee was monitoring work on a drain located 20 feet away from the valve when an unexpected blowout occurred. The force of the blowout propelled caustic material in the employee's direction, resulting in a slip and subsequent fall.

Employee was struck with metal screen and fell to the ground.

EE was standing on guarding rail 3 ft in air when rail broke; they extended left hand to break fall & crashed into the nearby wall w/that hand. EE ended up w/ scratch on L forearm, pain in L shoulder/R lower back. Worked rest of shift. Stiff/sore next day so went to Dr, was put on restrictions we can't accommodate, so lost time eff. 7/4.

Employee was laying a 4-inch pipe; when moving the pipe employees' pants leg was caught on a piece of PVC pipe, cause the employee to trip and fall forward landing on their forearms. Felt pain in their back a short time later. Employee was seen at clinic.

Miner misjudged the number of step walking down stairs, missed the last two steps, caught balance and returned to work area. Within 15 minutes they began having pain in upper thigh area and asked to leave work for the day.

Driller was attempting to put a grading in place when EE lost footing and slipped through the area around the head on the drill. EE hit just under EE's knee with the grading that was already in place. EE's helper helped EE out and EE went to the first aid kit; saw it was a deep cut and called Supervisor who in turn called Safety to assist. Went to the ER and received 10 stitches.

Employee was exiting loader, missed last step, caught EE's weight with right arm, did not fall, and noticed soreness in shoulder. required injected non-narcotic pain medication, hence reportability. further medical assessment scheduled

Miner personally in a hurry and tripped on a toe board edge properly placed. Miner fell and rolled onto scaffolding tubbing staged in the area. Miner received a lower back injury, wound/ laceration.

EE was unhooking a 3 inch flat water line from the water truck, and the EE stepped back and tripped and fell to ground.

Employee was walking section belt line when EE tripped on cable; tried to catch self and injured left hand/thumb.

I.E. was washing the water car at the dewatering sump on bottom. When EE finished washing the truck EE put the hose away and returned to the truck. While trying to get back into the truck I.E. slipped and fell twisting EE's knee.

Employee stated they were getting out of mantrip and slipped falling backwards hitting left elbow on the side of the mantrip. Employee sought medical attention on 7/2/2026 and was taken off work due to a fracture in the elbow.

EE reports they were starting a conveyer belt and it wouldn't start so they were walking check the emergency stop cables and they were walking down the line and they stepped in some slippery material and they fell and hit their butt and right elbow on a piece of concrete.

Employee was walking on a flat surface with no obstruction. EE stumbled and fell onto a non-slip metal grating. Injury to both knees and EE's left hand. The right knee required 14 stiches, and the left hand required three stitches. There were no broken bones. Employee required a tetanus shot. EE returned to work.

Employee cleaning dust off ground with air hose and lance, as employee stepped over truck guide, they tripped. Causing the person to make contact with a SafeRack pedestal with right knee.

Injured exiting 126 mantrip at 37 block Main West with seat in raised position, the seat fell and caught their foot causing them to fall and injure neck. Went to doctor on 06/30/26 and started missing work. No problems with equipment or compliance issues, training or PPE.

Conveyor went down due to being overloaded and they commenced to clearing the belt off to get back running when employee was repositioning feet they caused them to fall down on the rock surface below conveyor landing on their back onto the loose rocks. CT scans and x-rays determined they had compound fractures in their T1, T11 and T12 discs in the back along with a bruised lung.

While dismounting from the Caterpillar 775G haul truck, the miner lost their footing when on the ground and fell. Miner suffered sprained ankle.

While putting a conveyor inspection plate back up, it slipped and dropped to the ground and swept the feet out from under a miner causing the miner to fall onto their shoulder. This occurred at ground level.

On 6/24/2026, approximately at 10:57pm, an employee was working at our Pressure Deferential (PD) Rail Car unloading station when they tripped between track 4. The employee was wearing a hard hat as required, it came off during the fall and subsequently the employee struck the right side of their head on something. The employee sustained abrasions to the forehead area.

The driver stated while tightening the lug nuts, EE had pressure on EE's left foot and EE's right foot was 6-8 inches off the ground while EE was applying pressure to the breaker bar. At that time, the breaker bar broke and EE fell over sideways causing EE's right leg to break above the ankle.

Employee was working on a haul truck. When they completed working on the inside of the haul truck the employee stepped down and rolled their left ankle. The injury went reportable on 6/23/2026.

Weekend warrior slipped while walking down stairs from the plant feed belt, re-injuring lower back. Sought medical attention 6/28, missed 6/29, returned 7/4 missing 1 day of work due to a muscle strain of the lower back. No problems with equipment or compliance issues, training or PPE.

An operator exited the loader and was briskly walking to a control panel to shut off a feed hopper which had a rock stuck in the door when they rolled their ankle. The rolled ankle caused an avulsion fracture to the left ankle.

Employee was climbing down a steel ladder, after welding, slipped of the round rung and when they tried to catch themselves they struck their left hand on the ladder. After observation was taken to the clinic for x-ray and they fractured the fifth metacarpal and is pending procedure on 7/14/26

Employee went to kick a rock off the deck of a truck. Momentum caused them to fall backwards and got their arm entangled in the arm rails.

Employee fell to ground from CO# 1420 Cat D10T dozer while attempting to enter the cab. Employee sustained multiple contusions and abrasions and possible fracture.

After working on the wash screen, the team member was stepping down and stepped wrong and turn EE's ankle.

Employee twisted EE's knee while stretch wrapping a core pallet. Employee was taken to the hospital for evaluation, negative xrays and was told the employee sprained EE's knee and was released on restricted duty

Employee stepped on uneven ground, resulting in a trip and forward stumble. The employee landed on their right knee. Injury went reportable on 6/20/2026.

To the best of our knowledge no one seen EE fall. An EE saw EE climbing backup the berm after the fall. Then an EE ask of anyone saw EE fall. Everyone said no. We ask if EE was ok and EE said yes. EE did say EE's leg felt funny.

EE was standing, when they went to turn and start walking they tripped. EE sustained a laceration to left pinky finger when EE caught themself on a pie pan resulting in 3 stitches.

While traversing uneven, wet ground, employee lost footing. They used their right hand to brace themselves from the fall. In doing so, their right hand and/or wrist experienced elevated strain. Employee initially said they were okay, but a few days later sought medical aid.

An employee slipped and fell while changing the skirts on a conveyor. EE sustained a chipped tooth and a laceration to EE's lip.
