Miner was walking close to a front end loader when they stepped on something (possibly a rock) and injured their foot/ankle. their ability to walk was impaired.

The employee was descending a haul truck and got their boot stuck on the bottom rung. This caused their weight to shift and employee was unable to hold themselves and fell backwards (16 inches) onto the ground

Employee stepped off of a ladder rolled an ankle.

Employee was climbing down the ladder/steps of Loader #10 and the bottom step broke causing them to go to the ground hurting their right knee.

EE was descending the stairs to the office (4 steps) when EE's leg gave out and EE nearly fell. EE's right knee started showing

EE was cleaning in containment area, walking to get hose when they slipped. They fell and struck the pipes, injuring their right wrist. Seen at clinic, diagnosed with a fractured distal radius and released with restrictions site is accommodating. Referred to orthopedist, follows-up on 9/3.

The employee (Blaster) tripped over a rock concealed by muddy water, resulting in a fractured tibial plateau in right knee.

Employee was putting material on the office bulletin board. As they stepped backward, they tripped on a box of paper and stuck out their left arm to brace their fall. The fall resulted in a fractured humerus.

Employee was walking by a belt winder in the supply yard when the employee tripped on a piece of curtain that was being used to cover the belt winder. Employee fell and landed on their left hand. PPE was not a factor, mining systems was not a factor, compliance with rules and regulations was not a factor.

Employee was climbing down from Front Shovel when footing slipped and they held on with right arm and there was a popping noise in right shoulder and there is a tennis ball size hard knot on their shoulder. Employee has an AC joint separation.

Mechanic trip on a cord and fell to the floor. Left shoulder was injured when they hit the floor

Employee was working on the 9W CM Section. Was walking from the face to the auxiliary fan when EE tripped on a rock and fell down, landing on EE's SCSR EE was wearing.

Employee was hanging cable around the P.C on 14 section, employee stepped in a rut beside the high voltage tub and rolled EE's left ankle. Fracture to left fibula

Employee walking across location when EE felt pain in EE's left foot.

While walking to their truck in the gravel parking lot, the truck driver's left foot slipped and they fell landing on their right ankle breaking it.

Tripped on a pipe causing employee to fall and smash EE's hand resulting in 7 stitches to the right palm.

Employee was walking upstairs to breakroom and slipped resulting in their left knee twisting/buckling.

The Employee was operating a Pit Loader when the equipment experienced a hydraulic failure. Employee was injured while getting off the equipment.

Employee was stepping onto dozer, when their foot slipped off the step and they fell approximately 6" onto the push arm and broke a rib.

Employee walking out of change room fell on EE's lunchbox fell on EE's side hurting EE's left knee and left ribs.

777-Operator was dismounting a parked truck, misjudged the distance to the ground, rolled and twisted ankle upon impact and fractured lower leg bones.

EE was exiting EE's haul truck at the end of EE's shift. When EE stepped down to the ground EE stepped on EE's ankle wrong and fell down. When falling EE's right elbow hit the ground. EE was taken to OMP on 8/17/26 for further evaluation and placed on restricted duty.

EE entered the roller mill during a scheduled outage. When EE stepped from the platform into the mill EE rolled EE's ankle on the cylinder pocket. Coworkers checked out EE's foot and range of motion. EE's foot was swollen. EE was taken to the hospital and diagnosed with a hairline fracture.

while equipment operator was fueling the haul truck, stepped down with right leg, lost footing, and twisted right ankle. This motion resulted in an ankle twist.

Was working on dredge stern cable replacement from the deck of the work boat. As employee was walking from the operator's station towards the bow of the boat, they lost balance and fell into the mining pond. In an attempt to catch themselves, they hit their right arm on the boat causing an abrasion and bruising as well as mild lower back pain from the awkward movement of the fall.

EE was walking and carrying miner bits to mantrip when EE slipped on uneven and muddy mine floor, falling, landing on right hip.

Employee had walked away from EE's shuttle car when EE stepped awkwardly, twisting EE's right knee. EE was seen at the ER where EE was taken off work due to restrictions preventing EE from performing EE's regular job.

EE was assigned to cleanup along Fine Crusher 8 conveyor on 8 conveyor tripper floor. At ~6:45 pm, EE accessed the 8 conveyor on the south side and stepped on the dust seal belt and fell through ~12 ft. to the bin below. Resulted in fracture to 4 ribs, laceration to head and abrasions to torso.

Miner was sweeping out excavator cab standing on step while holding handrail with left hand maintaining 3 point of contact suddenly lets go and falls 16" inches to the ground below. Miner went to doctor with possible sprained back.

Employee was tramming the miner across the section when EE stepped into a hole, resulting in a sprained ankle.

Injured was on top of the miner in 8W retrieving a roof strap that EE dropped between the boom and the frame of the machine when EE fell down and struck EE's left knee on the machine, resulting in a contusion just proximal to knee, anterior side. No problems with equipment or compliance issues, training or PPE.

IE went through a mandoor to pull slack on a rockdust hose when they rolled an ankle on a rock.

Employee was coming down the 4ft ladder after greasing the pulley and missed the second to last step on the ladder. They landed on their left side and was diagnosed with fractured ribs.

A miner was walking in a slusher lane and hopped over a large rock. Upon landing EE felt EE's right knee painfully buckle. EE was treated for strain and released for light duty.

Employee working in the Marl Reclaimer control cab sustained a lumbar back strain after they were knocked against a wall and then to the floor, after it was struck by the Marl Stacker at approx. 0.351 meters/sec.

The employee was climbing over the beltline and EE's foot slipped on a bottom roller. This action caused EE to twist EE's knee.

Employee was exiting off of machine, stepped down onto step and foot slipped and fell to ground, resulting in laceration to the left eyebrow, received 12 sutures.

uneven walking surface. employee stepped onto a rock that caused employee to roll EE's leg. immediately EE felt a pop in EE's hip. employees hip was found to be dislocated (employee has had each hip replaced in the past).

Shovel mechanic was working on the drill mast and when stepping down they rolled their right ankle. The employee was placed on restrictions on 08/09/2026.

A mechanic was using crosswalk and fell to the ground landing on their left knee and elbow. They employee did not go on restrictions or lost time but did start physical therapy 08/24/2026.

CLP Employee reported knee pain after catching boot toe on a 4X4. Changed from First Aid to Medical Treatment at 08/17/2026 medical follow-up due to referral to physical therapy.

EE fell and landed on EE's face when EE was startled by an animal that jumped out. EE's eye was swollen from safety glasses impact on eye.

Slipped while climbing down from the skid steer.

miner was dismounting ladder of excavator when a foot slipped off the bottom rung due to mud and a finger was caught in block at the bottom of the handle.

Employee slipped and fell against a handrail bruising EE's right ribs

Security guard was spraying a Coal Truck off with a water hose at the truck wash on the Eagle Energy Haul road. As the employee was dragging the hose the hose rolled causing EE to lose EE's footing and fell causing a fracture in EE's right ankle. This injury was not a lost time and is only a medical reportable incident.

An EE was hand picking stone and placing in excavator bucket. Injured EE was observing. Injured EE just ate lunch in dump truck and was walking to where other EE was. Injured EE stepped on a log and slipped into excavator bucket with their side. Skin was not broken and there was no bleeding. We assumed it was just bruised. We later found out they injured a kidney.

Slipped when getting out of the loader on top step and fell to ground and landed on EE's back.

While mining towards the head on the 7W longwall face EE stepped over a relay bar and EE's foot slid on a piece of slick slate causing EE to fall and strike EE's left knee on a relay bar resulting in a contusion. No problems with equipment or compliance issues, training or PPE
