Accident type, without injuries

An unintentional roof fall was discovered on 6/30/2010 on the Main South, XC #20, between entries #3 and #1. The fall measured 110' L X 18' W X 5' thick. The area had been supported with 4' resin bolts. The fall will not be cleaned up. It did not effect ventilation, travel, or escapeway.

On the North A belt between cross cuts 14 and 15, an area of mine roof approximately 40 feet long and 20 feet wide fell above the roof bolt anchorage.

A rock fall has been found during a weekly examination at East-West Mains break #58 in the #1 entry, The fall area is adjacent to #25 seal, the size of the fall was approx. 30' x 25' x 5'. Additional roof support of cribs beams and steel arches will be installed in the fall area.

A roof fall was discovered in the No. 4 Entry of the 1 Main West set of Entries, approximately 30' outby survey station No. 879. The fall measured approximately 16 feet wide by 15 feet long by 8 feet high. There was no equipment involved. There were no injuries.

Mine examiner discovered a roof fall while making a weekly examination of the 8T #1 entry. The fall measure approximately 20'L x 20'W x 6' H. This area is part of the 8 Haulage Bleeder System.

An unplanned roof fall occurred in the Coalburg mine at spad #389 that measured 60' long x 20' wide x 7' high. No injuries were reported. The area was developed on 6-18-08.

A fall of roof occurred in the 2nd North East Parallel in the # 4 Entry, between x-cut #6 and #7. The fall was approxiamately 30ft long x 19ft wide x 6 1/2ft high.

An unplanned roof fall occurred in the #5 entry spad #1313. The fall is 1000' from the active face. Ventilation or passage of persons was not affected. Fall measures 75' L. X 20' W. X 8-12' H. Area was supported by 60"" torque tension bolts. All entries have been breakered off, area will not be rehabbed. Federal and State Agency's were notified immediately.

Found an unintentional roof fall on the #9 belt. Fall was 40' x 20' x 5'H.

Employee was inspecting the elevator when he inadvertently activated the governor causing the elevator to fault. Employee was entrapped on the elevator for 35 minutes. No injuries or damage to the elevator resulted from this incident.

The #1 elevator was out of service for a period of time exceeding 30 minutes due to a broken spring on the bottom door switch. The elevator went down @8:36 am. The switch was repaired and elevator placed back into service @10:16 am. No individuals were on the elevator at the time.

An unintentionall roof fall occured above anchorage at the Mains Section in the no. 7 face. The fall measured 7' high by 70' long by 20' wide. No injuries occured and No ventilation or travel was blocked by the fall.

A fall was discovered in the No. 6 entry of No. 2 Northwest at survey station no. 736. The fall was approx. 3500 ft outby the working section and measured approx. 7' high x 70' long x 20' wide. There was no equipment or injuries involved. The fall will not be cleaned up.

An unintentional roof fall occurred in the No.5 entry at spad No. 4636. The fall measured 7' thick, 19' wide and 40' long.

A roof fall was discovered in the intake during the weekly examine of the mine air comp by the examiner. The fall was approx. 13' wide, 15' long and 8-9 ft high. The fall was dinagered off and reported to the field office at msha-osma state.

At appx. 3:30 PM, an unplanned release of methane gas occurred in the No. 6 entry of panel 2A, inby survey station #1156, of the Northeast Seb-Mains of the above reference mine. A contributing factor to the release was a drop in barometric pressure.

5 breaks inby portal. Roof fall 3'5"" h x 8' w x 40"" l. Fell between bolts in an area of a hillseam. Roof is solid sandstone and this was a hillseam.

At 8:25pm the bearing on the governor rope to the service shaft elevator failed. An EE was on the elevator at the time this occurred. The elevator was brought back to the surface landing. The bearing was replaced and the service elevator was put back in service at 11:15pm on 06/28/2010.

It was discovered at 7:45 pm that some of the back and 4' split sets have fallen out of the back when the dayshift had blasted. Only 4 bolts fell out. It was the last round in the stope so the stope was shut down and we started backfilling and getting ready for sand.

A rock fall occurred in the B panel off of the pythian ridge submain at spad 7 + 20 #2 entry. Fall size is approx. 50 ft long, 5 ft high, and 12-14 ft wide. It is approx. 1400 feet from working section.

The elevator at the #2 Portal stopped with 1 employee on the elevator. Elevator stopped at 4:05 am, employee off the elevator at 4:43 am. Magnetic directional switch on elevator was found worn.

Roof fall above the anchorage in #8 heading 173 BLK 3 way intersection of 9 south. 30'l x 20'w x 10' high area was bolted with 8' bolts.

The South hoist drum brake spring was broken; management chose to make a repair. This was near end 3rd shift; allowed crews to exit during shift change. Repair completed; test done; 1st shift into mine at 7:40. North skip is for emergency evac; South skip had no effect; mine had 2 escapeways.

At 11:15 a.m. EE found a rock fall in intake of North Main at BK 22. This area is a paneled started and pulled back from. Fall was from #7 intersection to #6 intersections. Approx. 110' L. X 18' W. X 6"" H. Area is 2 bks inby spad 6812. Area timbered and dangered off. Ventilation still moving and no injuries.

An ignition of coal dust occurred inside of the #2 coal mill system which caused the two explosion vents to open and relieve pressure. A hot ember or material entered the coal mill causing the ignition.

A 20' cut had been taken in the #3 entry of 3 North inby the #22xc. When methane and water came out of the mine roof. Power was pulled and curtains were being used to ventilate section. Additional rock fell out of the unsupported roof exposing a methane feeder and additional water.

Ground Fall at the NHW 3520 Panel A3A6 - Miner noticed bolts in the rib taking weight, guarded area and called for shifter. While waiting for shifter a ground fall occurred at this area.

A fall was discovered at intake entry at 11:00 a.m.and was reported on same day. Fall was 20' X 20' X 8' and there were no injuries or equipment involved. Fall was 2500' from surface and 8500' from face.

Power line feeding production hoist failed causing a loss of power to the hoist. MSHA, State of Ohio, and CPP were all notified and CPP repaired line.

Lightning struck an oil storage tank at gas well location in the DIV area. The tank was destroyed and the oil ignited inside the containment berm. The fire department was not equipped to extinguish the flame so the well owners decided to allow the fire to burn itself out. The fire stopped at 10:00pm. No injuries or equipment damaged.

An unintentional roof fall occurred at the Galatia Mine New Future Portal MW Return, Crosscut 25R at 3349W, Entry #2. The fall was approximately 10 feet long, approximately 10 feet wide and approximately 10 feet thick. The fall did not affect ventilation or passage of men. The fall will not be cleaned up. Fall #27I.

Accident type, without injuries

On June 23, 2010 at approximately 7:15 pm three hay bales located in the M-5 area were noticed to be burning. The bales were located on the top of a hill; it is believed that a lightning strike from an earlier thunder storm that had passed through the area was the cause. No crews were working in the area when the fire was noticed; MSHA was notified because of the unplanned fire.

An Unintentional roof fall occurred 6000' from 001 MMU Intersection spad # 171 1st left 2nd South West Mains. Approx. 30'w x 40'l. x 8' h. Area was bolted with 6' tork tension bolts with 8' rope bolt in intersections. This area has been cribbed off and no plans to clean up fall.

Acetylene leaked from a hose into a metal box on the first floor of the prep plant and was ignited by a spark from a person burning a piece of metal on the second floor.

An unintentional roof fall has occured on the Main East at the 19xc of the #8 entry. The fall was approximately 45 feet in length and 10 feet high by 25 feet wide in the intersection. The entry width was 18 feet wide.

On the S-1 longwall set-up section, just south of survey spad 27479 in the face access entry, an area of mine roof 30 feet long and 20 feet wide fell above the anchorage point of the roof bolts.

A fall of the roof occurred in the SE Life of Mine Mains C at crosscut 175 in the supply road. The measurements of the fall are Length 20 ft X wide 16 ft X heigth 5 ft to 7 ft. Ventilation, equipment nor personnel were affected.

A roof fall occurred in the A-shaft bottom area. The area was between the whole switch and the back door switch, the area measured 25 ft long X 11 ft wide X 4 ft thick. The area had been mined in January of 1969 and original supported with 7ft conventional bolts. (MSHA was notified when discovered).

An unintentional roof fall occurred at the Galatia Mine New Era Portal on the 5 seam bottom area 750N 180W travelway going to the slope. The fall was approximately 60 feet long, approximately 18 feet wide and approximately 9 feet thick. The fall did not affect ventilation or passage of workers. The fall will not be cleaned up. Fall #27G.

A roof fall was discovered in 1A submain # 4 entry xc-15 spad #10 + 70 to the outby edge of xc- 16 spad # 11 + 40. In xc - 15 the fall is in #4 entry and goes through the intersection in xc- 5. The size of the fall is length- 160', width- 19' and height 8'.

Mine examiner found a roof fall in the North Mains Bleeders between 1 Right and 1 North. The fall was approximately 18' L x 18' W x 6' H.

A roof fall occured in the intake at break #68 between #8 & 9 entry at the mouth of the 1 Northwest section. The roof fall was 60 ft long, 18 ft wide and 8 ft high. Re-routed the intake, breakered the roof fall off. The roof fall will not be clesaned up.

Two longwall Coordinators were in the process of investigating elevated CO in the tailgate entry of the longwall. The CO occurred subsequent to a large cave in by the longwall shields near the tailgate corner. Elevated CO was detected behind shield No. 155 in the tailgate entry. Based on their findings a decision was made to evacuate the mine.

An unintentional roof fall occurred on the 10 right section at break 86 between No 1 and 2 heading. The fall measured 10 feet high by 20 feet long and 18 feet wide. There were no injuries, no equipment was caught and the ventilation was not disturbed. This fall will not be cleaned up.

An unintentional roof fall occurred at the Galatia mine New Future Portal at the #6 seam entry #1, 1400 N (approximately)(area had not been bolted either). The fall was approximately 18ft long, X 17-18ft wide X 6-7ft thick. The fall did not affect ventilation or passage of men. The fall will be cleaned up. Fall #27H

A roof fall was discovered by a mine examiner during his entry into 6-Right off of 9-Left worked-out area. The fall was located on the #5 entry 20 feet inby SS 1243. Cave dimensions were approximately 55-feet x 18-feet x 4 to 4 1/2 feet high. The area had been supported by 4-foot fully grouted resin bolts. An outby cutter may have extended into this area.

There was an unintentional roof fall on #2 unit's 3rd beltline. Length 100', Width 18', Thickness 5' . It was bolted with 48'' tension rebar bolts.

An unplanned roof fall occurred in the #4 entry at spad #1887. This fall occurred 350' from the active face. Ventilation or passage of persons was not affected. Area will be rehabilitated. Area was initially supported with 6' torque tension roof bolts and star pattern cable bolts. Fall measured 50'L x 40'W x 8-10'H. Federal & State Agency's were notified immediately.

Mine examiner discovered a roof fall at the #98 break on the White Oak Empty track. The fall was approximately 6' high x 14' long x 14' wide. The fall occured over top of an overcast and affected the ventilation in this area.

An unintentional roof fall has occured at the 15xc of the Main South. The fall was approximately 30 feet long by 10 feet high and 25 feet wide.

An electrical malfunction (blown 4160v fuse) caused a shutdown of the Production Hoist (part of secondary escapeway). Mine was evacuated; no problems, no injuries. Hoist back in service at 4:30am.

The elevator door close limit switch was not making contact which caused the elevator to stop. An employee was stuck in the elevator for 57 minutes. MSHA was notified via phone call. No injuries.

A roof fall occurred behind the substantial structure at XC-32 in the tailgate off LW-303. The fall occurred between No. 2 and No. 3 entries. The fall occurred in a worked out area, however the rubble from the fall knocked out a ventilation control, impacting the active workings. Because of this the fall was immediately reported.

An unplanned roof fall occurred at spad #143+50 to spad #144+90 in the #8 entry of the 3rd North Mains. An unseen slip was in the area. The fall measured 140' long by 19' wide by app. 6'-7' high. There were no injuries and ventilation and travel was not affected. This area is approximately 1000' outby #2 unit, MMU 002-0

On 6/18/10 at approx 5:15am, normal operating The #5 coal mill experienced a minor explosion. Upon inspection, a pair of plant pliers were found damaged in the reject chute wheel barrow, Pliers entered the mill unknowingly causing an apparent ignition of coal without warning. No damage and no injuries. Never stopped nor caught fire. The event was reported to MSHA @5:27am.

The fall was discovered on B-Sub Belt (#14 Block). The fall was measured at 7' x 16' x 30'. No equipment was damaged, and no ventilation disrupted. No people were injured as a result of this fall.

A roof fall was discovered in the 3 North 4 return aircourse, # 3 entry, at # 80 crosscut. The fall measures approximately 25' long 18' wide, and 6' thick.

An unintentional roof fall occurred in 1st Northwest main at 6:25a.m. The fall was in the #3 entry outby the face of the # 3 entry and 50'inby S 1379. Fall was 50' to 60' long and 17'-19' wide. Area was bolted with 6' super twist bolts in T2 channels with 12' cable bolts on 8' spacings.

The hoist would not work when called. The hoist was down from 2:15 a.m. to 6:35 a.m. At 2:32 a.m. the warehouse was notified regarding the hoist. At 2:33 a.m. MSHA was called with the information. The hoist was running at 6:35 a.m.

The miner crew was loading coal in the no.4 face in the middle lift cutting bottom when the ignition occurred. The cut was approximately 20 foot deep with .40% to .50% methane on the monitor read out. The flame was orange and blue in color and one fire extinguisher was used to extinguish the flame that lasted approximately 30 seconds.

A roof fall was discovered in the B-4 Returns at crosscut #8 in the #1 & #2 entries. The falls in each intersection measured 20' X 20' and was 5-7' in thickness. The area was previously supported with 5' Hy-Tech bolts and 12' cable bolts. The falls were dangered off and the returns will be rerouted.

A roof fall was discovered in a return entry of 2 left off B west. Fall was between survey stations #2723 and 2730. The fall was approx. 20' X 30' X 6' in height and will not be cleaned up. There were no injuries or equipment involved and ventilation was not affected.

An unplanned roof fall occurred 1 brk inby spad#42 in the #7 entry. Fall measured 40' X 18' X 6'. No injuries were reported. The area was developed on 5/19/08.

Shift foreman was riding down intake and found fall on old belt line. No equipment was involved and no ventilation was interrupted. Area was timbered and dangered off. Fall was 1 block inby spad 5523.

The man doors on the south skip were not properly secured by an employee at the end of 2nd shift. When the south skip was taken into the dump scrolls to test safeties, it became stuck on the ""A-Arm"" because of the open doors. The skip was damaged making it difficult to extract and interfering with the use of the hoisting equipment for several hours.

An unplanned roof fall occurred at spad #4413 in the #1 entry return. The fall is located approx 2000 ft from active section in the return airway. Area was developed Feb 2010. Fall measures 20'L x 20'W x 6'H. Ventilation & passage of person was not affected. Area was supported by 48"" resin bolts initially. All entries have been breakered off. This fall will not be rehabilitated.

A roof fall was found at 3pm on 6/13/10 at xc 3 travel-way along 1B belt, fall was 20X20X10 high in a 3 way intersection. Area had been supported with 8' full resin bolts. No injury resulted, discharge line and communication cable was lost. Fall will not be cleaned up. A new travel way has been re-routed around fall after additional support was added to access to fall.

The 4160 cable at the 3c-10 substation blew out and burnt the cable. As a pre-caution, the crew 11 men were evacuated. The team leader insured the crew was out and used a 20lb. fire extingusher to put out the fire. Electricians were called in to perform repairs. Incident occured at 1:15 a.m the evacuation was complete at 1:45 a.m. Mine was checked and cleared for reentry.

A roof fall was discovered on the main ""B"" conveyor belt. The fall extends from approximate belt location 2550 to 2635 and is approximately 6 to 8 ft thick.

A fall of the roof occurred in the South East Life of mine mains B on the 6th 54 belt at crosscut 162 to 163. No equipment, ventilation nor personnel were involved.

The man/material hoist became inoperable due to a malfunction in the braking system. A rod that applies tension to the brake itself broke. Repairs were made and the hoist was returned to normal operation at 12:35 PM.

It was discovered during a recent audit that a ground fall in M195-N1 headding that HAD been reported to the MSHA Immediately reportable hotline was not also reported on a 7000-1. This ground fall area has since been rehabilitated and hazards have been rectified.

Craftsmen were sent to add oil to #6 compressor at the air barn. Upon completion of service craftmen removed their locks and put the compressor back on line. Craftsmen were outside of the building when they heard a loud bang and then saw smoke coming from the air barn building. Supervisor and main gate guards notified and dispatched internal and external emergency services.

Rock fall occurred on 1 East Mains at break #29 in the #7 entry. Fall measured approximately 20' wide x 20' long x 6'-8' thick.

A rib rolled occurred at the intersection of West main #2 entry and #64 crosscut impeding passage in the alternated escape way. The section of rib measured 15'long X 5' high and 12' deep. The area had been rib bolted to plan with additional floor to roof support set. Immediate notification was made at the time the condition was discovered.

Rock fall occurred on 1 East Mains at break #27 in the #6 entry. Fall measured approximately 20' wide x 60' long x 3'-6' thick.

1500 level, 7C south face had several flames after blasting. After checking CO and methane levels, supervisor evacuated the mine and reported the fire to MSHA. The fire was extinguished on 6/12/10 with the approval of and in the presence of MSHA.

#2 hoist would not release the left brake. MSHA hotline called approx. 6:45 pm. ""MSHA"" returned the call and instructed the mine to call when the hoist was repaired. He was called approx. 10:05 pm. Electricians found a defective propotional valve control and replaced. The mine had access at all times to two routes of travel and escape via our #1 hoist and #2 skips.

Crew cutting bolt off discharge chute on the shaker with a cutting torch, rubber liner inside the chute caught fire due to slag being blown onto it.

High winds caused a high voltage line of Southern Cal Edison to arc causing the electrical power to the plant to drop. As the power dropped an employee was leaving his work area via a personnel elevator. The employee was not abe to exit elevator until power was restored. The power line arc also caused a grass fire on property that burned uncontrolled greater than 30 minutes.

A fall of the mine roof occurred in the 2nd SW Bleeder Panel in the #7 intersection of the #2 entry, two breaks inby survey station # 4356. The fall is approximately 10' thick, 15' wide, and 50' long. Fall was found by company personnel and an MSHA inspector during routine inspection.

An unplanned roof fall occurred in the #2 Gas Mine at brk. #3 in the #4 belt entry near spad# 1174. The fall measured 25' long X 20' wide X 15' high. No injuries were reported.

Foreman as walking the intake airway when a fall was dicovered @ 8:55 am. Location was break 15 #7 heading spad # 324. Fall was 65' long x 18 1/2' wide x 4' thick. Area was dangered off and cribs set at all entrance points. 5' torque tension bolts were installed previously in this area. Falling rock pulled some of the bolts from their anchorage point.

Discovered fall in the #4 entry of 1st north west mains section 25' inby survey station # 1371. The fall was 40' long by 30' wide by 12' to 13' high. Area was supported with 6' supertwist bolts, T-2 channels and 12' cable bolts. The fall was cleaned up and supported with 6"" I-beams and treated crib blocks. This rock was very laminate.

The hammer and starter rod fell 2 and 1//2 feet from the spindle adapter, the hammer (400 to 500 pounds) landed on the operator left arm pinning between the drill table and hammer. The operators' left forearm was broken.

Employee was shoveling from a pile of lime dust & it flushed down & over his boots. He went to clean out his boots & socks & treat his feet with vinegar to neutralize the lime dust. He then put back on his shoes & socks & returned to work. He later put on rubber boots & his feet sweated & the residual dust in his socks caused some burn area to both his feet.

Employee bumped elbow on corner of shelf causing numbness and tingling in fingers. Employee was under a doctor's care from the time of the injury to present. The employee had surgery on 2/11/2011 which is when the injury became lost time. Employee worked full duty until 2/11/2011.

The employee was driving a diesel ride down the road when he ran over a rock that was lying in the road. It flipped up and hit him in the left knee. The employee continued to work and his knee became worse until he could not work anymore. The employee started missing work on 10-25-2010.

Employee was working a head drive when a water line busted. He turned away from the water line and began to run away when he ran into a guide roller that was positioned beside of the belt line.

Employee was in process of pulling crabline out of water and placed hand in pinch point situation where his right wrist was pressed between the monkey line and boat, ultimately resulting in a bruised wrist.

Crew was spraying gunite on an overcast, when they had a plugged line. The crew located the plug, shut off the air to the hose and began to break the line. When the employee took the line apart, air and gunite shot out of the end strucking the injured in the face.

Employee drove a small endloader up to a spill at the east side of Granusol. It had rained hard all day. Employee stood on a wet surface as he turned to put a shovel full of ore in the loader bucket. While doing his normal job position, employee started having symptoms of lower back pain. After extensive testing the results show he is suffering from the occupational illness of degenerative discs. Per 2/5 document 220102230004, EE was doing walk around his truck, when he slipped and fell on flat ground.

Employee lowered a BO Stoper from the 3000N1E stope in a timber slide. When the timber slide was approximately 15 ft. from the bottom, the stoper, which was secured with #9 wire and a chain to the timber slide, dislodged from the can, fell onto a timber. The air operated leg then telescoped out, the pointed end of the leg struck the employee in the right shoulder and neck area.

He was initially diagnosed with a strain but a small tear was later found. He underwent a surgical repair and began missing time on 08/04/2010.

The employee was hauling coal with a battery operated coal hauler. As he was traveling down a haul road, he hit a hole in the mine floor causing him to strike the canopy of the hauler, straining his back.

The investigation is not complete, but as of now it appears a haul truck struck a pick-up truck. The cause of the accident is unknown at this time.

Operator of skid steer loader had his foot sticking outside the cage and brought boom down onto his foot. Pinched his foot (no broken bones). He has been told to not work for 7 days.

EE steam cleaned equip. until 3:30 pm fueled equip. and was returning to maint. area, EE stated he started down grade when truck jumped out of gear he applied brakes no brakes. The truck continued down the hill struck the berm and overturned coming to a stop on its top. EE was taken to hospital with injuries to right arm and several bruises. Released 06-21-10
