Accident type, without injuries

At 3:38 am an ignition occurred at the face of #3 Entry between 46 and 47 crosscuts in D 1 Section where the 14ED25 miner was cutting coal. The flames were from top to bottom the entire width of the face and were blue to orange in color. The fire lasted approximately 10 to 15 minutes. There was a visible clay vein making its way across the race from right to left.

The automatic doors for the 8 Haulage elevator, our secondary escape way, would not open when the elevator was at the bottom of the shaft.

It was discovered the brakes on the hoist would not release due to low pilot air pressure. Discovered automatic drain was stuck open and venting off part of the pilot air pressure. A new drain was installed and system tested and ready for use.

A roof fall occurred in the #2 entry along the 1st southeast mains at spad #44+20. The fall measured approx 18.5 ft by 7+ feet high and approx 21,444 ft from nearest section.

During a routine non destructive test of the hoist rope at the escape shaft, a guide rope was discovered to be bad. All miners were evacuated from underground using the main hoist. The guide rope was replaced and miners returned to work on their next scheduled shift on 4/30/2013.

The Sugar Run elevator was the scene of an entrapment of 3 people for approximately 90 minutes. A loose wire was discovered on the safety switch.

An unintentional roof fall occurred on the 1 North 1, Entry 2 between Crosscut 42 and 43. The roof fall measured approximately 60' in length, 18' in width, and 5 1/2' in thickness. A 103 (k) Order (8448493), was issued April 23, 2013, by MSHA Inspector.

An unplanned roof fall was discovered in the heading of 2 Right #1 Belt in the #5 entry at the #34 crosscut. The fall measured 15' long x 15' wide x 4' thick. The fall did not disrupt ventilation or travel. The affected area has been dangered off.

An unplanned roof fall occurred on the 1st Submain Southeast on MMU 001 at crosscut 5 in entry 1. Fall measurements 25 feet long, 8 feet high and 16 feet wide.

The Sugar Run Elevator was the scene of an entrapment of one person for approx. 2 hours 10' off the bottom landing, due to a loose wire on a contactor.

During the preshift exam a roof fall was found in the Primary Escapeway. The fall was reported outside and Msha was notified. The area was dangered off until inspected. The fall was approx. 80 feet long, 4-6 foot thick in the #6 heading. 55ton jacks and wooden timbers were installed to protect miners and the area.

Off shift blasting was conducted by the blasting crew. Heading 459BL1 was blasted and the blasting crew left the mine without incident. On coming post blast inspection was conducted by the oncoming post blast inspection team and it was recognized that a sulfide dust ignition had occurred during the blast. All gas readings checked during the post blast inspection were negative.

On 4-18-13 a roof fall was discover in the intake aircourse on the 2 South Mains at spad 1107. The intersection fell above anchorage that this location up to 6' high.

An unplanned roof fall occurred 40ft. inby spad 1393 in the #3 entry, along the 001 Section North East Mains. Measured approximately 20ft. wide by 16ft. long by 6ft. high in an area that had already been ""dangered off"", pulling 2 rows of fully grouted roof support. Found by examiner around 10:10pm and reported to MSHA's hotline immediately thereafter.

An unintentional roof fall occurred at break NO. 25 on the intake roadway of the No.1 Mains. The fall was approximately 6 ft. thick, 16 Ft. wide and 35 Ft. long

A Komatsu PC360 excavator was tramming towards a burning bush fire when it caught on fire under the cab. Forensic fire investigators have not yet determined the source of ignition. This is a non-injury accident.

Around 2 pm on 4/17/2013, Roof Fall was discovered in out by return air course. In the # 7 entry between spad # 930, and #1248. The entrance to the area was supported with timbers and dangered off. This doesn't impede ventilation. Does not effect airway examination.

The fall occurred on #1 South Panel (A Panel, #6 Block, #7 Heading)--between A3 and A4 inlets. The fall was 18' x 16' x 8'. The mine is monitoring A1, A2, B1, and B2 on a daily basis, and findings are being entered in the weekly book. The area was bolted with 6' tension and 12' cable bolts. The mine is preparing to seal the area.

A sand stone slab fell between #4 and #5 entries approx. 18' x 20' - #4 entry was cribbed at the intersection and had 8 Heintzman jacks set along travel way.

Haul truck was operated too close to berm and ended up on it's right side. Operator had no injuries. We do not believe this report is required in this case as there is no injury to report. We were issued a citation for not immediately reporting this even though an immediate report is not required. We were told by MSHA that this report was required to terminate the citation.

The air-compressor on the main hoist that released for the brake was determined to be bleeding past the check valve. New air supply was ran to the drake release and the miners were returned to work.

During the commissioning and start up of a new burner management system on the Autoclave Phase III 100K steam boiler, an un anticipated ignition of natural gas occurred in the forced draft induction inlet causing damage to the ducting.

A roof fall occurred in the 4 West area at 17 xcut in the #1 entry. The fall is outby the section. Dimensions 40 feet long, 30 feet wide and 12 feet high. The area has been re-supported.

Lost power to underground mine due to transformer failure related to weather. The mine was evacuated at 1:08pm. The transformer was replaced and the hoist was tested. Miners returned underground at 6:55pm

We had a roof fall in the #2 entry between the # 3 and #4 x-cuts of the Headgate #2 set-up rooms. MMU003 unit.

An unplanned roof fall occurred at spad #1054, in #1 entry in the old works. Ventilation and passage of persons were not affected. Area was initially developed in 2010 and was supported by 6' resin bolts. Length of the fall was 40ft long X 18ft wide X 7ft high. Area will not be rehabilitated and all intersections have been timbered off. Federal and State agencies were notified.

THIS ROOF FALL WAS DISCOVERED DURING THE WEEKLY EXAM OF THE ""OLD SEVEN ZONE"" AREA OF THE IMPERIAL MINE AT SPAD STATION No. 8699. THE FALL MEASURED 250'L X 18'W X 7'H.

A roof fall was found by fireboss in airway of 11 north, left return at 6cc of #1 entry (2 breaks outby spad 856). 20' wide X 70' long X 6-8' thick.

Began working in Cumberland Coal storage area (CCSA) Tue Apr 2. Smell indicated something going on but no fire determined. Previous week smoldering coal removed from ARJ storage side. Smoldering & red coals found in CCSA after access was made Apr 5. Crane used to move coal to center to use with kiln start Apr 6. Kiln didn't start. Open flame detected Apr 8 w/MSHA Inspector.

A roof fall occurred in the Main West in entry #5, at crosscut #102 at 86+10 foot mark. The fall was approximately 20 feet long, 20 feet wide and 12 feet high. The fall didn't effect the ventilation, or cause any damage to equipment. There was no injuries involved with this fall.

A non-injury water incident occurred on the 005-0 MMU when the continuous miner cut into a core hole in the 8 heading. This incident is being reported solely to prevent future enforcement actions due to MSHA's allegation that the water inflow was an inundation.

On 4/8/2013 a roof fall was discovered 150' inby the mine fan in the intake air course. The fall measured approximately 30' in length and fell up to 7' in height.

A roof fall occurred on the 19-W section at the 89 block location in the # 3 Intake entry. The fall is above the anchorage bolts, walkway is obstructed and ventilation not disrupted.

Operator was attempting to pack a rock out of the pit when the rock shifted to the left side of the bucket causing the loader to turn over on its side. The operator was not injured in this accident. This is for reporting purposes only. The operator worked the rest of his shift and operated an excavator.

An unintentional roof fall occurred in the 10th West Head Gate unit at #36 xcut in the #3 entry MMU 001. The roof fall was approximately 20' Wide x 22'Long and 17' Tall. The fall was cleaned up and a roof control addendum was submitted and approved for mining in no limestone areas.

The 8 haulage elevator, our secondary escapeway, had the processor for the brake system malfunction. The elevator was non operational for 52 minutes.

A fall of the roof occurred on the 2nd N.E. Sub Mains #1 unit crosscut 23 #4 entry 3 crosscuts inby the tailpiece, measuring 15' width by 10' length, 8'-9' height.

A roof fall was discovered in the #7 crosscut between the #5 & #6 headings. The fall was a continuation of a previous 2006 fall. The fall measured 40 x 16 x 5 to 6 feet in thickness. The area was initially bolted with 4 foot conventional bolts. The area was timbered off to prevent access and six 80 ton jacks were installed around the edges of the fall.

Two employees were trapped on the portal elevator from approximately 11:10 AM until approximately 3:50 PM. The cause of the incident was found to be a bad brake coil.

Pillar fall occurred and a mobile roof support became fouled to where it could not move on it's means or other means available.

A power surge on the feed line tripped circuit breakers to the hoist. The main external circuit breaker was reset but not the internal circuit breaker. A qualified person was sent from the mine site to reset the internal circuit breaker but due to travel time, the hoist was down for more than 30 minutes. When the internal breaker was reset at 10:10, the hoist was fully functional.

It was determined that the roof had deteriorated which caused the roof to fall to the mine floor. The fall was 70' long starting in the intersection of cross cut #20 along the 1st Southwest mains in the #5 entry going toward #4 entry. The fall was 18' wide and 15' plus in height.

Employee was cleaning a jammed rock from a return roller on a track mounted CEC Crusher. The engine was still running and the belt was engaged. Employee ""assumed"" belt was turned off because the head pulley was not spinning skipping on the belt; the belt was actually stalled. He did not lock out/ and tag the crusher before performing the work to de-energize all power.

The incident is still under investigation. The individual was struck by an unknown object when replacing guide rope bushings on the north skip in the loading pocket at the bottom of the 16 foot shaft. The victim was working on top of a skip, performing maintenance with 2 coworkers, when a piece of salt fell and struck him.

The crew was attempting to align a section of pipe, which is 40 feet long, when the 2 pipes disconnected and 1 end of the pipe that the crew was trying to install slipped out of the joint and struck the employee. An excavator was being used to position a 36"" pipe diameter by 40' long section pipe to connect it to another section of pipe.

Employee was unhooking a chain on the bushing fork of a drill. His finger was caught between the drill steel and the kick out arm roller. He was taken to the ER, diagnosed with a crush injury to the left middle finger. Transported to SLC where surgery was performed and a pin placed in the finger.

Off loading a drill hammer from a pick up truck by hand to ground. End of right ring finger was crushed/amputated when driver sub slid down to bit while lowering end of hammer to ground.

Employee placed arms in path of cargo lift wires.

Employee was cutting metal on hydraulic shear when distal right index finger tip was amputated by shear.

Over-exertion in lifting objects

While driving production haul truck #35, the injured employee attempted to move a rock approximately 30' X 30"" from the haul road.

Employee was loading bolts and glue into a scoop bucket when he twisted with materials in hand and heard a ""pop"" and felt pain in his back. He continued to work his shift but the following day, he went to the ER and was excused from work.

Was packing footshaft down steps and fell down footshaft hitting him on the wrist.

Fall from ladders

He put a 24"" ladder next to steering tire 42"" tall he was filling reservoir on radiator he asked driver if it was leaking that was standing next to truck he said yes there was. He went to get down ladder twisted side-ways he fell hitting his knee on steering lugs when I came in he showed me his injury and it was 4"" red spot. I asked him to go hospital he said no.

EE reports: tramming loaded SC to feeder and struck head on low-hanging belt clip. Resulted in neck strain. Treated as FA and under observation while performing required job duties. Pain migrated to lower back resulting in LWD beginning 06/14/2013.

Was building brattice wall Lifting a concrete block above his head and it dropped. Smashing his right index finger in between the concrete block and wall.

Individual was getting out of the loader and hurt his back.

Employee ran over a bump in the road and hit his head on the canopy. Employee stated that he was sick to his stomach.

Injured employee stated that while attempting to rotate the tail side shearer drum to change bits he felt pain in his back.

Employee was carrying jacks from a flat car to the belt line. As he stepped down off of the flat car he twisted his right ankle. This resulted in a fracture of the distal fibula.

Employee stepped in a hole and twisted his left knee and hip. Employee did not miss any work until 9/25/2013 when surgery was performed to repair a torn meniscus of his left knee.

Employee was walking from the face and stepped in hole causing him to fall to the rib.

EE WAS UNLOADING TIMBERS OUT OF A SCOOP AND WHEN HE LIFTED A TIMBER, HE FELT A SHARP PAIN IN HIS BACK.

Struck against stationary object

Employee was walking in the C&S basement and stepped on a piece of angle iron. He twisted his ankle. He was treated by a Dr. and at the end of eight weeks he still reported swelling and pain. He was sent for an MRI which showed the need for additional treatment. He was referred to an Ortho Dr. on July 2nd and restricted at the same time.

BMX employee was lowering his clothes basket at the beginning of the shift when the chain slipped through his right hand causing a laceration to the palm of the right hand.

Fall from machine

Employee was being loaded by the 495 Shovel and the bucket struck the right rear of the truck injuring the employee's left shoulder. Employee was transported to Logan Regional Medical Center and referred to a specialist. Specialist took the employee off work with a sprained left shoulder.

As Contractor was loading belt structure when a bottom roller fell off and hit his foot.

Miss stepped on grader latter causing operator to hold his entire body weight with his right arm on the entry handle so to not fall on grader blade. pulled his bicep muscle to a very painful state to which he could not curl his arm without pain level of 8 on a 1 to 10 scale. doctor ordered MRI may have torn muscle. MRI as of(5-2-2013)has not been done do to workmen's comp.

Over-exertion in pulling or pushing objects

While rolling a rail with a bar he felt pain in his right shoulder. He worked until 5-3-13 has been off for MRI. Result received for diagnosis was 5-28-13 and reported to consol on 5-29-13 for fluid build-up in shoulder.

Fall onto or against objects

He was getting in deck of roof bolter from the side. He put his legs in with his hands on top of the bolter deck. His hands slipped off hitting lower back area on the back rest of the bolter deck.

Absorption of radiations, caustics, toxic and noxious substances

Employee working on hydraulic cylinder he was unaware had pressure and was sprayed with hydraulic oil.

Fall from machine

Employee was climbing down from his haul truck. He slipped and fell about seven feet to the ground.

Employee was getting off equipment to replace a grease tube on hammer. While he was exiting he slipped on track falling. He had a hold of hand rail and held on to try to stop his fall. His body weight was on one arm and he twisted around hurting rt. shoulder.

Per initial incident report from supervisor: Mill employee was ""lifting bucket full of rock to dump onto the belt, turned a little awkwardly and strained his back"". Incident was initially reported as a first aid however a physical therapy report received on 5/16 for a visit from 5/15 indicates ultrasound and electrical stimulation was administered.

Struck by flying object

Employee was striking the lipsaver of the PC3000 Bucket with a sledge hammer during maintenance procedures and a piece of metal sheared off the lipsaver and penetrated an area of his lower left leg.

Bodily reaction, (Not Elsewhere Classified)

Employee was walking around the shop truck in the shop, when his right knee buckled. Employee managed to catch himself and did not fall to the ground. The cause of knee buckling could not be established, employees knee was not struck by or against anything, nor did he step on anything or walk on uneven ground. Incident occurred 15 minutes into the start of the shift.

Employee was trying to dislodge material accumulated between the belt of conveyor # 18 and the conveyor's structure with a metal bar. His right hand thumb finger was injured (contusion), when he missed the swing with the bar and struck the finger between the metal bar and on the plant's structural beam.

Employee was working in the BMX 2 A Headgate section. After the crew finished pulling some miner cable, he closed the cable arm towards the boom pinching his left ring finger between the boom and the cable arm.

Miner was changing a tooth on a CAT 990 loader bucket when it dropped on his foot, striking it along the edge of the steel toe. The miner suffered a broken toe on his left foot.

Rubbed or abraded

Employee was working on the S-1 conveyor to splice it. He had been working on his knees and his knees had been rubbing on the conveyor belt. He received and abrasion and subsequent infection.

EE complained of left shoulder hurting while shoveling #1 head and lifting parts that day. EE starting missing work on 2.25.2014 for possible RTC repair (acromioplasty)

EE was splitting stone. He stood up the next piece of stone he was going to split. The stone made some movement, slipped and smashed his 2 fingers against another stone.

Employee was operating scoop when he hit a rock. He hurt his back when he bounced into the canopy.

Flash burns (electric)

Mechanic had taken air compressor off line of electrical box to service. Upon placing the compressor back into service and back on live electricity a fuse blew.

Contact with electrical current

EE was wiring conveyor #11 starter which was locked out. This starter was directly above a 100 amp 480 volt 3 phase disconnect for conveyor #12 hoist and travel. This disconnect was energized on the line side. Screwdriver went into the open panel past the protective barrier and crossed phases creating an electrical explosion.

Employee was moving longwall flight when he was struck in the lower back by a timber that fell.

Over-exertion in lifting objects

He and another employee were carrying multiple couplings utilizing a pogo stick, he lowered the stick to his knee to drop a coupling and felt a pain in his lower back.

Employee lifted the tongue of a rock duster in order to pin it to his jeep. In doing so he felt a pull, a burning and stinging in his right shoulder. He continued to work the rest of the year and first of this year but started missing work on 2/10/14 to have rotator cuff surgery on his shoulder.

Employee stated he was cleaning up on patio there was a timber jack standing up against the building he went to move one jack and tripped falling on the storage box twisting lower left back.

Employee was lifting a pull chain onto top step of a loader, he said he felt something pull in his right shoulder. He was taken to the ER for E-Rays.

As Employee was pushing a loaded wheelbarrow it started to turn over. Employee attempted to counter balance the load when he strained his back.

Fall to the walkway or working surface

While examining gob vent borehole 59-2 he was approaching the site and stepped over a three-eights inch cable to gain access and tripped on the cable. He fell and fractured his right wrist and received a severe contusion on his left knee. The cable is used in lieu of a gate.

Injured employee stated that while pulling cable off the reel of a roof bolter he felt pain in his right shoulder.

Caught in, under or between a moving and a stationary object

Employee was rolling a PC2000 boom pin around on a pallet when his hand slipped off the edge of the pin. When his hand went over the edge of the pin there was a thin piece of metal exposed that went through his glove and the middle finger on his right hand. The employee was transported to the er, the metal was pulled out of his finger, drs opened the wound for complete cleaning.

He was rib bolting on the right side of the 8B miner. A piece of stone rolled out of the rib and struck him on the front of his right knee causing a contusion. He began missing work on 4/25/13.

Employee was walking down an inclined sidewalk, tripped and fell. Occupational Health Clinic nurse gave employee ridged wrist brace to wear.

Struck by falling object

Employee was changing the jack on a roof bolter operator canopy. While the jack and canopy assembly was lying on the mine floor, the sliding canopy extension fell to the mine floor striking the victim on the right little finger. This resulted in a fracture and laceration requiring surgical repair.

On 4/24/13 the employee tripped on grating and twisted his left ankle. On 5/17/13 he was evaluated by a physician and diagnosed with a fractured left ankle. He was given restrictions that prevented him from working. First day missed was 5/18/13. At a follow-up evaluation with a physician on 5/20/13, employee was taken off of work.

Operator was spreading sand from the bucket of a front end loader. The bucket struck the ground causing the loader to come to a sudden stop. The operator impacted the front windshield with his head and right arm/shoulder. The operator received treatment in the emergency room for the removal of glass and cuts to the forehead and right arm.

Contact with hot objects or substances

Employee was using a ""hand held"" welding torch to cut ""U bolt"" from the equipment. He failed to turn the torch ""OFF"" while he moved himself to a better position to complete the work. When he reached for the torch, he burned his hand.

Employee was helping install the plow on stage loader. The plow was held on by 2 pair of comealongs and 2 bolts. As the was being lowered the 2 nuts on the bolts popped off allowing the plow to twist and hit the employee on the foot. Employee was standing behind the plow.

Employee was assisting with the installation of a miner head when he stepped on a rock twisting his right ankle. He continued to work but went to the physician later that day and was excused from work. Diagnosed with a sprained ankle.

Employee was walking off the longwall face and twisted his left knee.

Flywheel struck employee in the chest causing him to strain his abdomen muscle. First ER visit showed no injuries from accident. A second visit to a Dr. showed a pulled muscles in the abdomen.

Employee was installing inside roof bolt when he saw a rock falling from the roof. He tried to get out of the way and the rock hit his right foot caused a contusion to the foot. As he was striking with the hammer, a piece of metal flew off, struck and imbedded in top of right hand behind thumb. Became reportable on 5/16/13 when he decided to have it surgically removed and lost 2 days work.

The employee stated that he and two other employees were lifting miner cable to place in hangers outby the face of #5 entry when he felt pain in his back. The employee sought medical attention and was diagnosed with arthritis and returned to work. Employee was removing a block wall. He pulled on door frame that was partially installed in the block wall and block started to fall. He was trying to run from the falling block and tripped backwards striking back of head and back. EE maintenance Worker for KCT, on 04/22/2013 at approximately 2:00 pm, was reconditioning roller frames on a 226 D9R Dozer. EE was driving the equilibrium bar pin out, and as it was 1/4 inch from being out, ee placed a bolt in it, and was holding onto the bolt when the equilibrium pin came out and the roller frame fell and pinched ee left thumb.

EE was unhooking a beam from the crane when he slipped and fell about 8-9 feet hitting his arm on a beam that was on the ground. EE was transported to the hospital for treatment and x-rays were done showing no broken bones and a dislocated elbow was placed on light duty.

Injured employee was assisting the scoop operator that was scattering joints of track rail using a chain with a bolt thru the end of the rail to pull with. As he was removing the bolt, the joint rolled over and pinched his left middle finger against the mine floor. He was taken to the local ER where he received sutures to repair the finger.

At approximately 1:38pm lightning struck the Volvo 110 Loader, #1443, on the lift arm or forks. It is believed that the lightning jumped from the forks and struck the ground employee on his right side and exited near his right ankle. The ground employee sustained burn injuries to his upper arm, right side of torso, right leg and foot.

Employee was assisting two other employees remove a belt clamp. The belt moved catching his hand between the clamp and belt roller stand. This is being generated pursuant to the Part 50 audit citation #8209368 issued on 8/4/2015. Management disagrees that a violation of the standard cited exists.

As he placed the drill steel against the top to drill a hole, a piece of rock fell from the mine roof, struck the drill pot, and then struck him on the left foot. Worked until 4/29, out for surgery.

Employee was building a metal cross over on the 2nd West Mains belt conveyor at cross cut #8. As he would tack weld the pieces together, he neglected to lower his welding hood and when he started to weld is when he would lower his welding hook. Employee sustained flash burns to both eyes.

He attempted to relieve the pressure by turning on the Ignition Key and cycling the Implement Pilot Controls. He then continued to loosen the Clamps when the D-Ring ruptured spraying Hydraulic Oil across his face. When descending the ladder he missed the bottom step and landed harshly on his Right Foot.

Employee was dismounting the 0-22 Cable truck from the passenger side when his right foot slipped off of the bottom step that is mounted on the fuel tank. EE will be off 8-12 weeks.

After getting the dozer loaded and exiting the machine, he accidentally stepped off the outside edge of the trailer falling approximately 4 feet to the ground landing on his right hip. Stated: Employee was opening mandoor between neutral and intake. The handle was stuck so he hit the handle with his right palm. After the 3rd strike, the latch gave way. He raked the outside of the little finger on his right hand across a burr where the round stock had been cut to make the handle. He lacerated his finger requiring 10 sutures to close. He remains off work.

Employee received an abrasion to the leg on 4/1/2013, while transporting an electric motor on a carte. Employee was transported to CAMC, where he was treated and release at this time. On July 10, 2013 received medical documentation that ee was treated and diagnosed for cellulites with chronic ulceration at CAMC. EE was give antibiotics and taken off of work.

EE was roof bolting when he was hit on his hard hat by roof rock. He was brought out of the mine for medical attention. he had an MRI which revealed Spondylosis at C5-6 and congenial fusion at C3-4. The MRI report is attached. he has not returned to work.

In the process of greasing the tail pulley of the conveyor, the employees glove came into contact with a speed sensor on the shaft of the pulley. The speed sensor caused his hand to be pulled toward the rotating shaft, resulting in a cut to his hand and arm.

On 4/24/2013, the employee went to move a barrel; the barrel had material in it he did not know of. He felt a slight twinge in his shoulder, but did not think much of it. On 8/22, a doctor placed him on restricted duty due to found tissue scarring in his shoulder.

We do not agree that this was a reportable injury. We do not accommodate ""restricted work activity"" at Excel Mining. Time sheets show he returned to his regular job the next day.

Employee was helping move the continuous miner from one place to another. He bent over to move the miner cable, raised it waist high when he felt a sharp pain through his right shoulder, neck and arm.

EE was using a step ladder to had oil to his loader. His foot slipped from the step and he fell from the ladder. He was app 3 foot from the ground when he slipped. He complained of pain and refused medical treatment. On Tues. 4-23-13 he requested medical for his knee and ankle. RedImed treated and put him on lite duty until 4-30-13.

He was evaluated by a physician on 4/21 and given prescriptions for a neck collar, pain meds, and released back to work with restrictions. Employee missed his 4/22 shift due to taking the prescription pain meds. He was scheduled off on 4/23 & 4/24 and returned with restrictions on 4/25.

Over-exertion in pulling or pushing objects

While lifting a rebar mat using a wooden 2x4 for leverage, employee stained his abdominal muscles.

Employee was replacing a light fixture, which was never properly fastened during original installation. Fixture began to fall during process of unhooking. From the ladder that he was working on, employee reached to catch the falling fixture. He suffered a torn biceps muscle. This did not become reportable until May 10 when he had surgery and was placed on restricted duty.

Caught in, under or between a moving and a stationary object

Employee was entering office building when she pulled the door open but didn't remove her hand from the handle as she entered the building causing her right hand middle and index finger to be wedged on the door puller. As she entered the building her fingers were pulled sideways.

Fall to the walkway or working surface

Mechanic was strapping a shovel drive shaft which was steamed cleaned. The drive shaft was on a pallet and Mechanic tripped on strap and pallet and fell to the concrete floor on left shoulder.

Employee was descending ladder to exit haul truck when he felt a sharp pain in his right knee. He sought medical attention and is currently on restricted duty.

Struck by falling object

Crew was pulling rods to change bit, the tube was stuck in the barrel so the driller got above the barrel to pull it out of the head and went to hand it down to the driller assistant, when handing the barrel to his partner, the barrel slipped from the driller's grip, dropping about four feet and landing the assistants right foot.

Over-exertion in lifting objects

Employee was hammering on valve and felt immediate pain to left side of groin area. Bulge was found over pubic area. Hernia confirmed on 4/10/2013.

Over-exertion in lifting objects

EE states that 2 weeks prior that she is having pain in her elbow from mopping, sweeping, and lifting. Sent to medical provider, and she is receiving physical therapy with no restrictions at this time.

Struck against stationary object

Employee bumped hardhat into sign and complained of neck pain later that day.

Caught in, under or between a moving and a stationary object

Operating scoop got left hand in pinch point and mashed two fingers on left hand forefinger and little finger.
