I NAME EITHER WAS TAKING OFF MANWAY COVER OR REPLACING BOLTS ON MANWAY COVER WITH IX COLUMN STILL UNDER PRESSURE AND INLET AND OUTLET VALVES STILL OPEN. MANWAY COVER BLEW OFF AND STRUCK HIM ON FRONT RIGHT SIDE OF SKULL.
