A FATAL ACCIDENT OCCURRED AT THE #3 MINE CANADA COAL CO INC. FROM A FALL OF RIB IN WHICH THE VICTIM I.NAME AGE 30 WAS CRUSHED AGAINST THE RIPPER HEAD ON A LEE NORSE 265 CONTINOUS MINER THIS OC CCURRED IN THE #3 ENTRY 100 FT OUTBY THE FACE AREAS WHILE THE EMPLOYEE WAS INSTALLING BITS
