EMPLOYEE WAS SW3TCHING A CAR OF SUPPLIES IN THE #3 ENTRY TRACK HAULAGEWAY AND RATHER THAN APPLY HIS BRAKES HE REVERSED THE LOCOMOTIVE AND OPENED THE CONTROLLER WIDE OPEN THIS ALLOWED THE LOCO MOTIVE TO SLIDE3WITH VERY LITTLE BRAKING EFFECT THE LOCOMOTIVE TRAVELLED TO THE BOTTOM OF THE GRADE AND STRUCK A PARKED PERSONNEL CARRIER WHICH WAS SETTING AGAINST ANOTHER 8 TON LOCO.
