WORK WAS UNDER 4AY TO REPAIR A SPREADER BAR THAT HAD BEEN REMOVED FROM A DRAGLINE BUCKET THE BAR CONSISTING OF A TWO 8 INCH DIAMETER SCHEDULE 80 PIPES AND STEEL BLOCK AT EACH END WAS ABOUT 3 30-35 FT LONG I7 HAD DEFORMED IN THE SHAPE OF A 1 SUCH THAT THE BLOCKS WERE NOW 15 FT APART BAR WAS BEING HELD UPRIGHT BY A FORKLIFT A WELDER HAD CUT ONE OF THE , IN PIPES NEAR ONE OF THE BLO
