HE WAS A FOREMA3 AND HE WAS GOING TO MOVE THE WILCOX ROOF BOLTER FROM #2 TO #1 AND HE WAS APPARENTLY RIDING THE ROOF BOLTER AT THE CONTROLS WHEN THE ROOF BOLTER WENT OVER A HUMP AND HIS KNEE HIT THE ROTATIO3 AND UP-PRESSURE AND IT CAUGHT HIS HEAD BETWEEN THE TOP AND THE HEAD ON THE ROOF BOLTER
