VICTIM WAS ADJU8TING CONVEYOR IDLERS ABOVE GROUND 75'-80'. A SAFETY BELT WITH 2 LANYARDS WAS BEING USED BY VICTIM TO PROTECT AGAINST A FALL WHILE PERFORMING THE REQUIRED WORK. VICTIMS LANYARDS *%<<%*<*<%<*%<*3 VICTIM UNTIED LANYARDS TO MOVE TO NEXT WORK POSITION WHEN FALL OCCURRED.
