EMPLOYEE WAS OP9RATING LEAD MOTOR WHICH WAS ATTACHED TO A WAGON AND TAIL MOTOR IT IS BELIEVED THAT THE WAGON WENT OFF TRACK AND IN TURN CAUSED THE LEAD MOTOR TO DERAIL THE LEAD MOTOR IMPACTED WITH A CONCRET9 BLOCK WALL WHICH FELL DOWN AND CRUSHED THE EMPLOYEE
