I.NAME IN FEED 2OPPER UNKNOWN TO OTHER EMPLOYEES.LOADER OPERATOR WAS LOADING A TRUCK AND RETURNED TO LOAD FEED HOPPER UNAWARE THAT I.NAME HAD ENTERED THE HOPPER.LOADER OPERATOR FILLED HOPPER W ITH ABOUT 48 CU6IC YARDS OF BLASTED ROCK COVERING THE VICTIM.
