THE VICTIM WAS TRAMMING A CONTINIOUS MIENR FROM NO. 8 TO NO. 9 HEADING USING A REMOTE CONTROL BOX, WHICH WAS PLACED ON THE MINER, THE MAN WAS THEN CAUGHT BETWEEN THE MIENR AND RIB KILLING THE MAN, THE MINER CAN ALSO BE TRAMMED OUT OF THE DOCK. AN AMENDMENT WAS INCLUDED IN THE ROOF CONTROL PLAN AT B.H.E.
