EMPLOYEE WAS LOADING A RAIL CAR WITH CEMENT DESPITE BEING GIVEN A SAFETY BELT BY HIS LEADMAN & INSTRUCTIONS TO USE & DESPITE WARNINGS BY WITNESSES TO PUT IT ON IMMEDIATELY PRIOR TO THE ACCIDEN T HE DID NOT DO SO HE TRIPPED ONTO THE RAIL CAR & FELL INTO THE HATCH HE LATER DIED OF SUFFOCATION
