DRIVER WAS DUMPING LOAD OF SHOT ROCK ON STOCKPILE. EMPLOYEE FAILED TO OBSERVE UNDERCUT STOCKPILE. STOCKPILE MOVED AS EMPLOYEE WAS DUMPING. TRUCK FLIPPED OVER BACKWARDS LANDING ON ITS BED AT THE BASE OF THE STOCKPILE. DRIVER ALSO FAILED TO USE SEAT BELT AS INSTRUCTED. MAN DIED 7-8-91.
