WHILE SERVICING A JEFFERY 101 CONTINUOUS MINER, INJURED BECAME ENTAGLED IN THE CUTTING HEAD WHEN THE CUTTING HEAD ACCIDENTALLY CAME ON, PULLING HIM UNDER THE HEAD, FATALLY INJURING HIM.
