CRUSHER WAS ROTATED WHILE EMPLOYEE WAS STILL IN CHAMBER. INJURIES TO EMPLOYEE WERE FATAL. EMPLOYEE WAS ASSISTING IN FREEING A LARGE BOULDER FROM THE CRUSHER.
