THE EMPLOYEE WAS OPERATING A LHD UNIT IN THE 990 NO ORE DRIVE WHEN A GROUNDFALL OCCURRED RESULTING IN A FATALITY.THE INCIDENT WAS DISCOVERED WITHIN APPROX 5 MINUTES AND RECOVERY OPERATIONS BEG AN IMMEDIATELY.THIS VICTIM WAS PRONOUNCED AT APPROX 11:50 AM BY P.A. BILL CARLE OF THE BEATTY MEDICAL CLINIC.
