VICTIMWAS ASKED BY CONTTRACTOR TO CUT OFF NUTS TO 20 TON COUNTERWEIGHT FOR REMOVAL & TRANSPORT AS PER CONTRACT AS VICTIM CUT LAST OF TWO NUTS COUNTERWEIGHT SWUNG TOWARD HIM PINNING HIM BETWEEN CONTRACTORS GEN POLE WINCH TRUCK & THE WEIGHT VICTIM WAS CRUSHED AS HE STOOD ON HIS LADDERR TRYING TO ASSIST CONTRACTOR
