THE VICTIM WAS STANDING APPROX. TWO FEET INBY PERMANENT ROOF SUPPORT OPERATING THE CONTINUOUS MINER VIA RADIO REMOTE WHEN A PIECE OF ROOF ROCK APPROX. 7FT X 7 FT X 5-0 INCHES FELL ON THE VICTI M CAUSING FATAL CRUSHING INJURIES.
