THE DRAGLINE OPERATOR WAS PERFORMING DAILY OPERATIONS WITH A 1978 NORTHWEST MODEL 190D DRAGLINE, WHEN THE POND BANK CAVED IN CAUSING THE DRAGLINE TO FALL INTO THE POND IN APPROX. 40 FEET OF WA TER.
