THE DRAGLINE OPERATOR WAS PERFORMING DAILY OPERATIONS WITH A 1978 NORTHWEST MODEL 190D DRAGLINE, WHEN THE POND BANK CAVED IN CAUSING THE DRAGLINE TO FALL INTO THE POND IN APPROX. 40 FEET OF WA TER.

EE was working on the Dewatering Wheel when their wrench slipped and struck them in the chin. EE sustained a small laceration and was taken to the ER to be evaluated where they received 3 stitches.

Employee descended the stairs and stepped to the ground, turned, stepped on ice, fell extending his left hand to grab the handrail. He instead struck a crusher wear liner laying on the ground lacerating the palm of his left hand. He was wearing all required ppe including leather gloves. He received 5 inner and 14 outer sutures.

This person is NOT an employee of Crawfordsville.He is a truck driver for A&C Trucking.The driveway was paved over weekend,the stairs to the scalehouse had been removed. Trk Driver had stacked 2 blocks in front of the door. When trk driver went out the door, he missed the step, fell on his left elbow. Had swelling-pain. Received a sling,was restricted, returned to work sameday

EE alleges he fell off a front end loader on 10/27/10 sustaining multiple injuries including a closed head injury during his course of employment. EE did not report this on date of alleged injury. There were no witnesses. Supervisor denies there was an injury. Employee filed application for adjustment of claim on 1/5/11. Employee was terminated on 11/1/10 for attendance issue.

Employee was taking the bucket teeth off of the dragline bucket. He was using a hammer to dislodge the bucket tooth when a piece of steel from the tooth chipped off and went into employee's right thigh. The piece of steel was embedded in his leg and he has to have surgery to have the piece of steel removed.

Employee was fueling the fuel truck from the stationary fuel tank when he stepped from the block wall into the truck and slipped and hit his right leg. Thought he would be o.k. A few days later his leg started to swell. He had received a hematoma/contusion to his right lower leg.
