EMPLOYEE WAS INSTALLING A BOOM TO A BELT DRIVE, WHEN A CHAIN HOIST (1 1/2 TON HARRINGTON) , CHAIN BROKE CAUSING BOOM TO SHIFT TOWARD DRIVE ROLLERS, STRIKING EMPLOYEE ACROSS BACK AND RIDING HIM DOWN, RESULTING IN A FATAL INJURY.
