THEY WERE REPAIRING (PACKING) IN A LIFT CYLINDER JACK WHEN THE I-BEAM HOLDING THE BUCKET APPROX. 12' OFF OF THE GROUND SPLIT DOWN THE CENTER ALLOWING THE BOOM AND BUCKET TO FALL TRAPPING HIM. THIS RESULTED IN AN IMMEDIATE FATALITY.
