EMPLOYEE WAS WORKING ON COARSE AGGREGATE WASH PLANT. BLADE MILL CRUSHED EMPLOYEE. REPORT WAS DELAYED DUE TO TRANSFERRING OF REPORTING RESPONSIBLITY AND INTERNAL MISCOMMUNCIATON. VERBAL REPO RTING WAS COMPLETED IMMEDIATELY (1-8-97).
