THE LOADER OPERATOR WAS ASSIGNED TO LOAD MATERIAL FROM A STOCKPILE IMMEDIATELY ADJACENT TO A CRUSHER FEEDER. FOR SOME UNKNOWN REASON, HE BACKED THE LOADER DOWN THE SLOPE 2/3 OF THE WAY TO THE WALL OF THE ACTIVE PIT, WHICH WAS OUT OF HIS ASSIGNED WORK AREA.
