THE LOADER OPERATOR WAS ASSIGNED TO LOAD MATERIAL FROM A STOCKPILE IMMEDIATELY ADJACENT TO A CRUSHER FEEDER. FOR SOME UNKNOWN REASON, HE BACKED THE LOADER DOWN THE SLOPE 2/3 OF THE WAY TO THE WALL OF THE ACTIVE PIT, WHICH WAS OUT OF HIS ASSIGNED WORK AREA. HE WENT OVER THE ACTIVE PIT FACE, FIRST HITTING ON THE RADIATOR END OF THE LOADER AND THEN OVER ON ITS TOP TRAPPING HIM INSIDE
