EMPLOYEE WAS IN PROCESS OF INSPECTING THE PRODUCTION SHAFT WHEN HIS LANYARD APPARENTLY HUNG UP, RESULTING IN EMPLOYEE SUSTAINING FATAL INTERNAL INJURIES FROM PRESSURE RECEIVED FROM SAFETY "O" RING BELT.
