EE fell while putting away tools at end of shift. (Had been installing permanent handrail.) EE fell from first floor (steel floor 16' 9" above ground floor - cement foundation); multiple injuries. Had removed his harness at end of shift. Fell through safety tape that was installed between temp. & perm. handrail. Died on 2/23/05 as result of injuries.

Getting cables to put together to pull pipe out of water. While putting clevis on pipe, pushed the clevis too hard and it gave way mashing left hand index finger in between clevis and pipe. The result is a broken finger.

Our employee was traveling in a "jon" boat out to a dredge. The boat had a small outboard motor attached to it. The throttle on the engine malfunctioned and the boat did not decelerate as anticipated. The boat then struck a ladder attached to the dredge. Upon impact, our employee fell forward, striking his knee on the deck.
AN 18" PIPE WAS BEING CUT ON PIPE STANDS. WHILE TURNING THE PIPE IN PIPE STANDS TO CUT BOTTOM SIDE OF PIPE, THE PIPE FELL ON EE'S RIGHT FOOT. PIPE STANDS ARE FOR 12" PIPE MAX.
