Struck by falling object

EE was removing pin from cylinder on 214 rock truck. EE left pin hanging in the eye to reposition self to finish removing and during this process the pin fell sticking EE on right middle finger.

Employee was buckling two suction hoses together. EE was using a 12 inch crescent wrench as leverage. It slipped off the buckle and hit EE in the right side of EE's forehead.

Struck against stationary object

Employee was changing out a turbo on a Hitachi Excavator. The ratchet (hand held tool) EE was using broke causing the hand to strike the turbo flange lacerating the pinky finger.

Employee was descending from a bulldozer while servicing it. EE's foot slipped and EE's leg got caught between the pusharm and track. EE fell over and fractured a tibia in their right leg.

Employee was climbing down form a rock truck. They slipped and fell.

Employee was operating a rock truck. A large rock was loaded onto the truck. The rock rolled from the loader bucket to the truck bed shaking the cab causing the employee to hit the cab framework.

Lead Blaster was loading wet bags in bore holes directly from pump truck and experienced pain in lower back.

Employee was getting off a rock truck. EE slipped and fell.

Employee slipped getting off a loader. EE had contusion of the right knee.

Employee was operating a rock truck in which rocks were being loaded. A large rock was dropped by the loader onto the truck and it caused a jolt. The employee's back was hurt as a result of the impact.

Employee was passenger in straight truck that tipped over on its side due to poor and muddy road conditions resulting in employee bruising left shoulder and a contusion to forehead.

Employee was line boring/machining a hole with line boring equipment when EE somehow got arm and hand caught in the boring machine causing a fracture to the left arm and ring finger.

Employee was climbing down from dozer. The wind blew the employee down. EE fell on the push arm and was pinned for a few minutes.
