While using A-frame ladder to remove material from elevated beams, the RAM team member had already made several trips up/down the ladder. On way back up for another trip, ladder tipped over while EE was climbing, resulting in a fall. The RAM team member was taken to the hospital. Medical evaluation, including x-rays, revealed a fracture to both the tibia & fibula, requiring surgery
Metal plate had a burr causing a laceration to EE right hand.

Employee was building scaffold over conveyor belt. Employee slipped 18 inches catching armpit on the scaffold bar. This dislocated EE's shoulder. Shoulder was put back in place same day.
Miner drove a loader across feeder #1 when material had been removed. The plant operator had triggered feeder #1 and failed to communicate to the loader operator. The miner was injured as the wheels on the loader dropped into the feeder, causing miner to experience multiple body strains.

While getting loaded the loader operator jarred the 777 haul truck that the employee was operating straining their neck and back. Employee's official notification date is in question. The official notification was given on 5/26/2021

Driving over bumps near the face causing EE to bounce around in the cab of the haul truck. Note: Initial treatment did not involve reportable treatments (restricted duty or prescriptions). Employee was released to full duty. On a follow up visit on 8/13/2019 employee was given prescriptions and was recommended therapy

NOT WORK RELATED. Just after EE got to work EE began feeling chest pains. EE notified the safety coordinator. EE was transported to the Office to await emergency medical service. EMT's arrived the pain had decreased and the onsite EKG showed normal stat's. EE was transported to the hospital under the advice of the EMT's.

Installing large bull gear on roll crusher at Wash Plant One, using a 8 lb. hammer to strike a hammer wrench to secure the nut that holds the gear. Hammer glanced off wrench and hit left knee.
Removing a rock from Grizzley chute with a come a long. Chain slipped off the rock and rock smashed pinky finger.

Removing bolts on a truck differential, two bolts broke off as he was pushing and pulling and pulled a muscle in back.
Employee was repairing a power cable (S/O 480v) that was pulled out of strain relief. He was moving the cable to get slack for the repair. There was cut or worn spot that shorted out causing an arc that caused first and second degree burns on right hand and thumb.

Went into the screen to clear a jam in the rock chute. Was getting out of the machine onto ladder, foot stuck and pivoted foot resulting in a strain/sprain to left ankle.

EMPLOYEE WAS LOWERING THE LINER INTO THE CHUTE WITH THE CRANE, THE LINER WAS A LITTLE TOO WIDE, EMPLOYEE STOPPED THE CRANE OPERATOR & WAS TRIMMING THE LINER. THE LINER SLIPPED DOWN THE CHUTE & THE UPPER END CAME UP PINNING THE EMPLOYEE.
Employee was changing a tool bar on a D9N dozer and hurt his right shoulder; told his supervisor his arm hurt, not that it was from the job. Never stating anything else, he then went to the doctor on 4/21/04. This is when he states it wasn't getting any better. Safety department received the paperwork from the job site on 4/29/04 to file this MSHA report.

WHILE INSTALLING A NEW GEAR BOX EE WAS HAMMERING WHILE A CHIVE 2 FT ABOVE HIM ON THE FRAME. HIS LEFT ARM WAS ON TOP OF THE GEAR BOX WHILE HE WAS TIGHTENING THE BOLTS WITH HIS RIGHT HAND. CHIVE FELL ON LEFT WRIST.

EE WAS IN PROCESS OF CHANGING THE BLOW BARS IN THE IMPACTOR AND THE ROTOR AND BLOW BARS ON ONE SIDE HUNG UP AGAINST THE WALL. A CRANE WAS HOOKED UP TO HELP TURN THE ROTOR AND APARENNTLY WHILE THE CRANE WAS LIFTING AND PRY BAR WAS BEING USEDHIS FOOT SLIPPED DOWN INSIDE AS THE ROTOR TURNED AND IT CAUGHT HIS RIGHT FOOT.
