Beating on bucket teeth with a hammer in order to loosen for rotation. Piece of metal sheared off of bucket teeth and hit mechanic in upper left arm. Metal was lodged in arm and had to be removed.

Ignition switch on pump engine not working properly. EE tried to turn crankshaft to get flywheel & starter in time. He grabbed fan with l. hand & pushed fan belt in with r. hand. The engine started with key in off position & pulled his r. hand fingers (index & middle) in between fan belt & crankshaft pulley. Severed fingers (index: middle phalange; middle: distal phalange).

Employee while splicing belt (BC2) climbed onto cage being used as a tool platform and proceeded to walk to the edge. The platform became unbalanced and fell 7 - 8 ' with employee on top, to the ground. Employee suffered injuries and was treated on 9-21-05 thru 9-24-05.
