Anchor cable was being moved. Employee was pulling cable as it was coming off winch. Employee complained of soreness in his back Monday --- Informed me that he thought it happened while pulling the cable.

Miller Brothers Sand & Gravel Inc · Fall onto or against objects

Employee was working on assembly of conveyor belt assembly. Assembly slipped or shifted and dropped down less than 1 foot. Employee lost balance and fell backwards. He struck his forehead on an unknown object and received a cut on his left eyebrow. Employee was transported to medical facility and received 13 stitches. Employee has returned to work.
