EE WAS REMOVING A CUTTER HEAD WITH A HAND POWERED JACK. THE JACK SLIPPED AND STRUCK HIS LEFT LOWER LEG. HE SAW A DR. AND RETURNED TO WORK. HE SUFFERED SOME DISCOMFORT AND STARTED PHYSICAL THER APY ON 8-11-98. HE CONTINUES TO WORK HIS REGULARJOB.

EE WAS ADJUSTING UPPER SPUD PIN. HIS RIGHT INDEXFINGER WAS CAUGHT BETWEEN THE PIN & THE SPUD COLLAR. THE SPUD THEN DROPPED & THE PIN STRUCK HIS RIGHT FOOT.
