While mounting tires on rims in shop - felt pain in left elbow.

While replacing recoil spring on feeder for crusher the tooling slipped causing the spring to pinch left thumb.

Employee was picking rocks by conveyor and got finger caught between rocks, cutting finger on sharp edge.

While getting off dragline, EE twisted knee on rock on the ground.

While putting air in tire in the shop, the tire sidewall blew out causing the tire to flip over knocking the employee down causing the tire to land on right ankle.

Employee caught knee between tire and bumper while putting on alternator on Truck. Put pressure on knee and or twisted knee cap.

While working on 2400 Dragline air cylinder, the wrench slipped which caused EE's wrist to hit the brake band/drum.

While using pry bar on Lippmann Crusher, bar slipped hitting EE's right palm of hand. This caused cut on palm of hand. EE was not wearing gloves.

While working on crusher, pry bar slipped and cut finger.

Putting on welding hood when something got in eye.

While removing a rock from the crusher, the rock rolled and hit employee's right knee.

EE slipped getting our of bobcat skidstear loader. He tried to catch himself and jammed his hand into the ground

While walking on catwalk at end of day, employee slipped and fell on right side.

Employee was trying to get rock out of the crusher when he slipped, tearing finger on guard when falling.

A steel bar used to clean the feeder fell against the door window of the control room. Employee was inside the control room at the time. A piece of glass or dirt went into his right eye from the broken window.

Door of loader blew closed on right hand causing injury to middle and ring finger.

WHILE WORKING ON LIPPMANN 32X36 HAMMERMIL, FAMMER FELL PINCHING INDEX FINGER AGAINST ROTOR. EMPLOYEE DID NOT WAT TO GO TO THE DOCTOR WHEN THE ACCIDENT HAPPENED BUT DID GO HAVE IT LOOKED AT THE NEXT DAY

WORKING UNDER TRUCK. DIRT FELL IN EYE CAUSING IRRITATION.

FIRST REPORTED ON 3-18-03. WHILE CHANGING HYDRAULIC LINE ON EXCAVATOR, HYDRAULIC FLUID SPRAYED ON EE'S FACE. SKIN REDNESS AND IRRITATION OCCUREDON LEFT SIDE OF FACE. WENT TO DOCTOR, RECEIVED A PRESCRIPTION AND TOLD TO STAY OUT OF THE SUNSHINE FOR A DAY.

WHILE WORKING ON PTO/CLUTCH, CLUTCH DISC SLIPPED OUT OF HAND CUTTING LEFT MIDDLE FINGER.

WHILE REPAIRING TIRE, RIM FELL ON RIGHT ANKLE AND FOOT.
