Contact with electrical current

Employee was kneeling at control panel conducting voltage test. Voltage was good. EE was attempting to close the panel door when they noticed insulated wires were out of their channel. EE proceeded to push the wires back into the channel at which time EE was shocked in the left hand.

Absorption of radiations, caustics, toxic and noxious substances

EE was attempting to jump 380-7 Komatsu with the 320 Komatsu. EE was told by other senior employee, to wait for other EE to finish loading a truck, for other EE's assistance. EE proceeded to connect cables, unassisted, and incorrectly attached cables to battery of 380-7 causing the battery to explode. EE was wearing PPE but was exposed to acid in EE's face and eyes.

The outside salesman, an employee of E.R. Jahna Industries was Standing and walking in open field where trucks weight in and out, evaluating customers trucks turn around time from mine to job site and back. The salesman started to have a hard time breathing and collapse.

EE was working on dredge and got overheated. The initial signs looked like a mini stroke, so was called into MSHA. After doctors visit, doctor stated it was brought on by heat exhaustion.

Employee was bent down on conveyor, holding onto a bracket while greasing a roller, when he lunged forward and hit his head on another bracket cutting his face above his right eye and hitting his right arm.

Was standing on bottom step of ladder,his foot slipped off ladder ,causing him lower back pain.

Standing on slurry pipe taking roots out of clean out, as he was getting off pipe, his foot slipped out from under him. Landed on his butt, causing lower back pain.

Fall from machine

Loader operator was going forward not knowing a car was behind him. Operator went into reverse, car driver tried to reverse, Operator accidently hit his car. Driver opened his door, fell out of car onto pavement hitting his head.

While cutting a piece of electrical wire, he was pulling the box blade knife upward to cut the wire. His hand and box blade knife slipped, cutting his upper right lip. ( Recieved Stitches Only)

Employee was sitting on pipe while cutting cable off pipe. The pipe had too much tension on it and when cable was cut the pipe rolled and employee fell in water.
