Employee was grinding a piece of metal when the end fell the off grinding table and the employee attempted to catch it heard a "pop" in left shoulder.
Employee was assisting in pulling multiple power cables by hand when employee heard a 'pop' in left shoulder then felt pain.

Employee struck left arm on poly pipe as ee lost balance and fell off workboat.
Employee while walking down stairs slipped and fell. Grease gun they were carrying landed on left index finger causing lacerations.

While working on the Dredge shaker screen with another employee, putting rubber conveyor belt on for skirts. Employees were pulling the conveyor up to clamp around support beam when Employee felt something pop in his right bicep muscle area. The piece of Conveyor Belt weight approximately 134 lbs.

OPERATOR WAS CHECKING FOR BLOCKAGE IN THE SUCTION SIDE OF PUMP SHELL AND CUT LEFT MIDDLE FINGER ON ROOT BLADE INSIDE PUMP. EMPLOYEE RECEIVED TWO STITCHES ONLY, RETURNED TO WORK SAME SHIFT.

Lifting a pipe with a chain hoist, chain slipped and the pipe smashed the left ring and middle fingers. Employee had choice to have finger grafted to chest for three months or have amputated at first knuckle, Employee decided to have middle finger amputated at first knuckle.
Climbing bank out of pit, twisted knee.
Slipped and fell on steps hurting left rib area, tailbone and left ankle.

Operators were fusing some poly pipe together and they were moving the hot plate of the fusion machine. The operator grabbed the hot plate by the sides and not by the handles, receiving 1st degree burns to both hands.
Discharged line on drill rig got clogged, Operator was tapping pipeline with hammer, the clog came loose, and the pipe line lost control and hit operator on left side of face, causing laceration. Received stitches.
Employee was replacing swing cable on dredge, working on bank of pit and stepped on unlevel ground hurting left knee and calf muscle area
Operator was getting off bulldozed and slipped and fell landing on soft sand
