Employee stepped off of a ladder and while backing up, employee tripped over crusher blow bars on pallet. Employee fell on the blow bars causing a scrape on right shin.

While picking up spreader bar to put on lowboy, spreader bar for the dragline bucket. The spreader bar slipped, pinning finger against a piece of metal.

While climbing out of dial split tank, EE heard something pop and pull in left shoulder. Pain became worse as time went on.

Employee twisted/overturned left ankle getting off ladder on Pioneer crusher.

Employee was using a hammer, striking a back-out hammer (a punch hammer), when a piece of metal broke off the punch end, striking leg.

EE was trying to loosen nut on Eagle Sand Screw. Nut broke loose. In process the wrench pinched finger against frame.

Employee was taking engine out of loader when engine broke free of transmission. Right elbow hit loader frame. Continues to have soreness in elbow. Employee did not report accident until 1/13/2017.

Employee was pulling linkage pin on a 980G Loader. Pin shifted smashing/pinching tip of left little finger.

Employee was moving Pegson crawler crusher with hand control. The door to crusher panel hit ramp structure. Door support cylinder knocked his hard hat off and door fell down cutting head.

Employee was working on the 375 Excavator, when loosening a hydraulic line, oil sprayed on left hand and body.

EE was helping to repair #1 Belt on the Aggregate Plant when he felt a burning on the inside upper thigh. He assumed he was stung by a bug. Date first reported to supervisor 12/23/13

Rock from crusher hit employee in mouth.

EE lost control of truck and hit power pole. He does not know why he lost control.

EE was taking back cover off engine compartment of 980B when cover slipped and cut and pinched left little finger on frame of loader between cover and frame.

EE was digging out the 3500 dragline shovel. He felt pain in lower back. Pain now runs down back of legs.

EE was working on clamps on Pioneer belt. A sliver of metal entered eye. EE was wearing glasses and face shield.

Employee found tick on right side of neck. He pulled it off. Wednesday morning he found a lump on his neck and it kept getting bigger.

EE foot slipped when he was walking on dragline. When he went to catch himself, his left hand grabbed a mirror which broke resulting in a cut middle finger.

Accident was not reported until January 16, 2012. EE was lifting cribbing when he felt a pain in right middle of back.

Employee slipped and fell on left knee at Canica Crusher.

When employee was crossing under conveyor he slipped on the wet ground. He tried to catch himself. All weight went onto left ankle when he fell.

A side arm on 740 Articulated Haul Truck was being cleaned in wash area when the arm slipped off blocks falling on right foot.

This accident was not reported to supervisor until 1/22/09. Employee was operating a 740 haul truck. He complained of hitting a rut in road and was thrown around in cab. Seat belt was on. When it was reported to supervisor, employee complained of blood in urine. Then when he went to Dr. he complained of blood in stool.

Another employee was throwing a chain over an overhead pipe when the hook on chain hit him on forehead.

EE was taking out back plate of #2 screen deck, when the plate fell, pinching outside of right foot.

Repeated constant use and writing of weight scale tickets.

EE was tightening teeth on dragline bucket with 3/4"" drive ratchet when he pulled back muscle. Did not tell supervisor until 10/26/07.

Shoulder sore from swinging sledge hammer while helping to repair a broken drive chain on 2400 Dragline No. 2.

EE was driving a d400 Haul truck when he hit a bump and felt something in shoulder. Pulled muscle in left shoulder.

Employee was pulling on a wrench when the wrench slipped off bolt. Wrench in his hand came back and hit him in the face.

EE was walking between crusher and catwalk when he stepped off side of catwalk causing ankle sprain. (overturned ankle). Did not want to go to dr. on 8/10/07. Then 8/23/07 he decided to go to Dr.

While cleaning crusher, ee strained his shoulder and arm.

Ee was greasing the truck bed cylinder on a ladder. The ladder started to fall. Instead of falling with the ladder, He jumped off the ladder and landed on his feet. He felt some pain in his lower back.

Did not want to see doctor when it first happened. Went on 12/1/06. EE went to remove a 10-15 lb rock from conveyor when he felt a pull on lower back.

While opening the lid to the dumpster, to dump trash, employee felt a bite between right thumb & 1st finger.

Employee got out of the F450 Service Truck to jumpstart the F350 truck. He said that he set the parking brake but was unsure if the truck was in gear. He went to the front of the truck. While trying to open the hood the truck rolled forward pushing him against the F350.

While inspecting a rock jammed in crusher, a rock hit his face shield causing a cut on the forehead.

Rock bounced off crusher and hit employee in the right eye.

While walking into shop, employee tripped on the floor mat and fell. She said she was fine & refused medical treatment. On 8/10/06 she said knee & arm hurt and wanted to see a doctor.

Slipped by wall while cleaning clamshell bucket.

EE HAD EPILEPTIC SEIZURE. HIS MEDICATION HAD RECENTLY BEEN CHANGED. WHEN SYMPTOMS WERE NOTICED HE WAS REMOVED FROM JOB SITE. EE WAS IN EMPLOYER'S TRUCK BEING TAKEN FROM MINE PROPERTY WHEN SEIZURE BEGAN. NO INJURIES OCCURRED.

KNEE ALWAYS HURTS. WHEN EMPLOYEE WAS COMING DOWN LADDER, HE STEPPED ON GROUND FROM LADDER AND KNEE FELT LIKE IT WOULD GIVE OUT.

EMPLOYEE SAID HE SLIPPED & FELL ON 1/25/05. HE NEVER TOLD THE SUPERINTENDENT ABOUT THE ACCIDENT UNTIL 02/01/05. EMPLOYEE SAID HE TOLD (2) CO-WORKERS THAT HE FELL BUT THE (2) CO-WORKERS DENY HE TOLD THEM. HE WENT TO THE E.R. ON 2/01/05. THEY SAID HE HAD A SPRAINED BACK & HE WILL BE OFF WORK UNTIL 02/05/05.

While walking down steps of the Pioneer Primary Crusher EE slipped. Foot came out from under him. caught himself but his leg fell on the step cutting leg.

FIRST REPORTED ON 3/16/04. EE WAS MOVING OPERATOR'S SEAT TO SERVICE MACHINE WHEN HE LOST HIS BALANCE AND FELL AGAINST HANDRAIL. DID NOT WANT TO SEE A DOCTOR UNTIL 3/25/04.

EE SLIPPED AND FELL ON BOLT WITH LEFT LEG WHILE WORKING ON CAT WALK ON CRUSHER.

EE WAS RUNNING THE LOADER WHEN A BEE FLEW IN AND BIT HIM ON LEFT RING FINGER.

EE WAS HOLDING A BOLT FOR COWORKER TO TAKE OUT. WHEN COWORKER GOT THE NUT OFF HE HIT BOLT WITH A HAMMER. BOLT HIT EE IN FOREHEAD.

GREASE GUN FELL TO GROUND AND UPON EXITING MACHINE TO PICK UP GREASE GUN RIGHT ANKLE TWISTED.
