Struck against a moving object

Employee was operating 982 CAT Loader at the South Plant when EE went to get a load of concrete sand to load a customer truck EE felt pain in EE's lower back. Operator was wearing seatbelt when incident occurred.

Employee was stepping from work boat to deck of dredge when foot slipped on the wet deck and landed on EE's face. Employee received 13 stitches to lower lip, and minor cuts and scraps to the facial area. Employee released to full duty, reported due to receiving stitches.

Over-exertion in wielding or throwing objects

Employee had a loader bucket on a goose neck trailer, and threw a chain and binder over the bucket to chain it down. When employee threw the chain employee felt a share pain in employee's right elbow and forearm.

Over-exertion in pulling or pushing objects

Employee was reinstalling a impeller on pump and was pushing it with employee's foot to line it up so they could get it on the shaft and employee felt a popping sensation in employee's lower left back area.

THIS WAS A CUSTOMERS TRUCK THAT WAS DRIVING TOO FAST FOR CURVE AND TURNED OVER ON RIGHT SIDE. DRIVER RECEIVED LACERATION TO RIGHT EYEBROW. SPEED LIMIT SIGNS, 25 MPH, CURVE SIGNS AND ARROWS WERE ALL IN PLACE, Date of birth is unknown, SSN is unknown.

Employee was closing storage door on the A-frame of a 28 foot trailer and the steel lid slipped out of EE's hand, pinching off the soft tissue of EE's left ring finger.

Employee was cleaning roots out of suction, bank caved in and hit end of boat throwing operator into water. Operator stated he did not want medical treatment at time of incident. Took a weeks vacation came back and went to Doctor on 10/11/2005. Was put on light duty, but paper work was not misplaced until 10/25/05. Employee is still on light duty, reason report filed late.
