Struck against stationary object

EE was operating a John Deere 410E Articulated Truck. EE was traveling from the pit to the stockpile area to dump a load of clay used to make brick. EE was distracted, ran off the left side of the road on to a berm. The back bed of the truck rolled over. EE was not wearing EE's seat belt. EE was thrown out of the seat. EE's head was cut requiring 7 staples.

The dozer operator got off dozer to talk to a truck driver. The operator stepped on a lump of kaolin, rolled left ankle. The operator had a fracture of the left ankle.

We were contacted by an employee that one of our miners thought he was having a heart attack. We went to the scene to find a Mack Dump Truck operator grabbing at his chest, complaining of chest pain, pale skin, profuse sweating, and numbness of the face. We immediately took the driver to a local hospital. He was given a EKG. He was diagnosed with having a panic attack.

Using an impact air wrench to install a bolt in a bush hog wear plate. Using left first and middle fingers to hold the bolt in place when bolt spun, lacerating both fingers.

Employee picked up some metal along the outside rear of the shop building. The metal had poison ivey or poison oak growing on it. Later in the day, employee noticed a rash on both arms. He was diagnosed with contact dermatitis due to plants.

Co-workers were using chain saws to cut sections of a fallen tree. Ee was picking up the sections, which had vines on them, and toting them out of the way. The next day, he noticed a rash on his right forearm. Since then, the rash has gotten larger and has fluid blisters.

EE had climbed onto a pile of logs. The log he was standing on rolled out from under him when he turned. EE fell and landed on his right elbow.

While working on a side mower gear box, EE felt something on his face and knocked it off. The next morning, he noticed his right forearm was swollen and he noticed a yellow head had come up.

AFTER TIGHTENING AN ALTERNATOR BELT ON CATEPILLAR 623E SCRAPER, EE WENT TO STEP OFF OF THE MACHINE. WHEN HE DID, HIS LEFT ELBOW STRUCK THE PAN. HIS SKIN WAS BROKEN AND BLED FOR A FEW MINUTES. NEXT MORNING HIS ARM WAS SWOLLEN AND HURT.

EMPLOYEE WAS USING A STRING WEED-EATER CUTTING GRASS & WEEDS ALONG MINE HAUL ROAD WHEN HE WAS STUNG OR BIT BY AN INSECT ON HIS UPPER BACK, LEFT SIDE.

EE WAS OPERATING MACK TRUCK #12 PULLING AN ALUMINUM DUMP TRAILER. THE TAIL GATE ON THE DUMP TRAILER WAS STUCK SO EE STRUCK HIS HAND INSIDE OF BOTTOM OF GATE AND PULLED. THE BOTTOM PART OF GATE INITIALLY OPENED THEN JERKED BACK, CATCHING EE 'S HAND BETWEEN GATE AND TRAILER. EE S RIGHT LITTLE FINGER WAS DIAGNOSED WITH A FRACTURE AND A LACERATION.

EE HAD ANNUAL HEARING TEST PERFORMED 6-11-03. RESULTS RECEIVED INDICATED A STANDARD THRESHOLD SHIFT. EE HAD EAR INFECTION IN BOTH EARS. RETESTWAS PERFORMED ON 7-3-03 WITH RESULTS STILL INDIC ATING A STD THRESHOLD SHIFT. EE'S WORK AREA TESTS BELOW 85 DBA. EEHAS WORN EAR PLUGS PROVIDED BY COMPANY SINCE 1999.

EE WAS OPERATING #5 MACK DUMP TRUCK. HE WAS NOT WEARING THE SEAT BELT WHEN RIGHT FRONT WHEEL HIT A POTHOLE IN THE ROADWAY BESIDE CLAY STORAGE SHED. EE S HEAD HIT TOP OF CAB AND HIS LEFT KNEE S TRUCK THE STEERING COLUMN, WHEN HE WAS THROWN UPWARD.
