Struck by falling object

While attempting to change cheek plates, a plate fell, glanced off the left foot of the EE and landed on the right.

Fall from machine

Tech #1 was on top of a drilling machine inspecting the engine. Tech #2 was on the ground at the truck looking for a tool. Tech #1 yelled for tech #2. Tech #2 ran over just as tech #1 was falling. Tech #2 reached out and grabbed tech #1 enough to slow EE's fall but not keep EE from hitting the ground. Tech #1 hit the ground head first then the L side of EE's body hit the ground.

Struck by falling object

Employee was changing spring on a deister screen. They were holding the screen with a cable and crane. The cable broke and the screen dropped causing something to strike the employee in the chin. The employee required stitches and repair to two teeth.

The driller was in the process of removing the DTH hammer from the drill steel. He applied a locking wrench to the hammer, and he stepped out of the cab to connect the drill steel to the rotation head. As the two parts connected, the wrench turned and struck the driller's foot, fracturing bone in his foot.

The injured employee was attempting to unwedge a stuck conveyor roller bracket, when it moved and pinched the inside of his left little finger between the frame of the conveyor and the roller.

Struck against stationary object

Employee was helping clear crusher platform. As he swung his arm to throw a rock, his hand struck the handrail, causing a fracture to his finger.

Accident type, without injuries

The loader operator was pushing in the surge pile at the secondary plant. At approximately 16:30, the loader engine shut off and would not restart. The operator parked the loader and left to get the mechanic. Upon return, he found the loader on fire. The fire was extinguished within 40 minutes.

Injured employee and another were attempting to drive parts out of a drill hamer casing. One employee was hold a 1"" diameter bar and the other was striking it with a hammer. The hammer glanced off the bar and struck the employee's hand.

Struck by falling object

Employee was assisting in removal of conveyor frame from old secondary surge tunnel. After support legs were cut loose, the frame moved, striking the employee's hip, causing a dislocation.

ON NOV. 26, 2004, THE EMPLOYEE WENT TO THE LOCAL E.R. COMPLAINING OF BACK PAIN. HE CLAIMED HE HAD INJURED HIS BACK WHILE AT ANOTHER QUARRY ON JULY 17, 2004. PLANT MANAGER AT BOTH LOCATIONS STATE HE DID NOT REPORT ANY INJURY OR ACCIDENT AT ANY TIME BETWEEN JULY AND NOVEMBER. EMPLOYEE CLAIMS TO HAVE INJURED HIS BACK ATTEMPTING TO MOVE A LARGE ROCK FROM THE HAUL ROAD.

INJURED EE HAD RAISED HINGED GUARD TO GREASE CONVEYOR TAIL PULLEY. WHILE GREASING, A GUST OF WIND BLEW THE GUARD DOWN, STRIKING EE'S HAND.
