While attempting to remove a pipe fitting the pipe wrench slipped, striking them on the top of the left foot

The employee never reported illness to supervisors. Left workplace. Later said, sick due to the heat. Requested medical excuse from the employee, so the report could be filed if applicable. Employee failed to provide to date 8/02/2023.

Chain smashed fingers on a right-left hand. The accident occurred on the ledge. EE was pulling stone. EE had fingers between the chain and the clamp. EE was flown to Augusta.

Employee was using a scaling bar tool when the tool slipped and crushed EE's right index finger on the automatic thermal burner frame.

Working on 3900 Manitowac Crane using a 24"" crescent wrench. Adjusting a 1-1/4 bolt on brake band. The wrench slipped and mashed the tip of right thumb on a metal guard.

Going back down ladder after rain to ledge #1. Foot slipped and bounced down ladder to bottom 8-10 foot. Broke left arm between wrist and elbow. Bruised right hip.

Working on ledge #3, lost balance and fell forward. He tried to catch himself with his hands. He hurt his left hand.

Pulling (picking it up) drill steel for the line drill. To put steel in hole.

Running drill, changing steel out and latch hit on left knee.

Picking up rock on ledge #1.

Cleaning up shell rock from ledge.

Pulling on dog chain.

Exiting loader, possible fall.

He was trying to pull a board out of tool box when he hit his left arm on top of tool box inside top.

AIR HOSE WAS BEING HOISTED OUT OF HOLE BY CRANE. THE HOOK BLOCK WAS GOING BACK AND FORWARD. WHEN EE REACHED TO GRIP IT, IT PULLED ON RIGHT ARM.

CHAIN DOGS HIT EE ON RIGHT SIDE OF HEAD ABOVE CHEEK.

HE CRUSHED HIS LITTLE FINGER PICKING UP WASTE GRANITE ON THE LEDGE.

LOOSENING BOLT ON BOOM OF CRANE. WEIGHT OF WRENCH SLIPPED AND FINGER CAUGHT ON METAL.

CO-WORKER WAS WORKING ON PIECE ON CRANE AND INJURED WAS HELPING HOLD. HOT SLAG FROM CUTTING TORCH HIT LEFT EYE.

BLOCK DOGS HIT RT. SIDE OF HEAD, NEAR RT. EYE.

DRILLING ROCK WITH PLUG DRILL. SOMETHING WENT INLEFT EYE.

SERVICING ENGINE ON CRANE. SLIPPED ON CRANE HIT BACK OF RIGHT SIDE OF HEAD. RIGHT SIDE LOWER BACK.
