Over-exertion in lifting objects

EE was first injured on 3/12/18 but never reported the incident. While squatting to lift a tipped over 50gal drum EE felt pain in the groin. On 9/5/18 EE first reported the incident. EE waited another month before seeking treatment. It was discovered EE had a hernia. The operation was performed on 11/19/18. Due to the delayed nature of this injury it is now being reported.

Was repairing big sifter leaks and when I picked up the door and turned I felt an instant pain in my groin and abdomen area. It felt like a strain or pulled muscle. Was diagnosed as a hernia 12/8/10. See Attachment.

Checking for spark on furnace and it ignited and caused a flash burn on right eye.

Was using hand drill, drill bit got stuck in the hole. Drill turned, pinching hand between it and a bolt, cutting finger.

Was using an acetelyne torch, the torch sparked back after being shut off burning left hand.

EE was cleaning molds and leaned to the left to scrape the mold. There was a metal pole behind EE, two molds caught & lifted up which caught EE between the pole and the mold under my left ribs. EE twisted & pulled herself out.

WORKING ON A TWO-LEVEL CATWALK HE STEPPED IN A HOLE, LOST HIS BALANCE AND FELL THROUGH. HE HIT HIS ARM AND RIBS ON CATWALK; FELL APPROXIMATELY 6 TO 7 FEET. BROKEN RIBS, BRUISES, SCRAPES.
